Provat denied that he accepted Workers Compensation or Injury claims. I was able to find him through that search criteria of WOrkers Compensation . Extremely unprofessional staff , walk in office , so over crowded . He charged my regular insurance, since he is non-network provider , the cost was deducted from my annual deductible limit. I will not recommend anyone to visit him.
