Q: Are you accepting new patients and at what ages do you see patients?
A: Yes, we are accepting new private pay patients. Our Medicaid panels are closed, so we are unable to accept new Medicaid patients. We see patients from birth to 18 years of age.

... Do you have separate waiting rooms available for well children?
A: Yes, we have a sick child waiting room and a well child waiting room. Only children coming in for well child exams and physicals may sit in the well child waiting room. This to help prevent from getting our well patients and newborns ill.
Q: When should I bring my newborn in for his/her first visit?
A: Your newborn should be seen between 3-5 days old for a jaundice check. If your child is not jaundiced, then your newborns first visit should fall between 10-14 days old for his/her first well child exam.
Q: How often should my child get a Well Child Exam/Physical?
A: Well Child Exams start at 2 weeks of age then 2 months, 4 months, 6 months, 9 months, 12 months, 15 months, 18 months, 2 years, then every year thereafter on or after your child’s date of birth.
Q: At what ages does my child need immunizations?
A: Your child should get his/her first immunization at birth while still in the hospital. Your child should receive childhood immunizations at ages 2 months, 4 months, 6 months, 9 months, 12 months, 15 months, 18 months, 2 years, 4 years, then every 10 years thereafter. For immunizations at 12 months and 4 years your child’s appointment must be scheduled on or after his/her date of birth, no earlier.
Q: Can I obtain a copy of my child's immunization record?
A: Yes, you may come by our office to obtain a copy of your child's immunization record. However, there will be a $3.00 charge per patient. We cannot fax any immunization records. You can ask for a copy at your child’s appointment to receive the record free of charge.
Q: My child has a daycare/physical form that needs to be signed by the doctor. How do I go about getting this done?
A: You may drop off your form anytime to be signed by the doctor. There will be a $5.00 fee for completing all forms. You must make sure the patient/parent portion of the form is complete before dropping it off. Forms will not be signed otherwise. Also, these forms will only be signed if your child has had a Well Child Exam (Physical) in the last 12 months. Please see Downloadable Forms tab to the left and click on Medical Form Policy.
Q: What is the offices referral policy?
A: The patient must be seen by one of our physicians before a referral to a specialist can be obtained. Please allow 7-10 business days for your referral to be processed. Most referrals require an authorization number from your insurance company which can take up to a week for our Medical Assistants to obtain. If an urgent referral is needed due to an emergency situation, we will do our best to obtain this referral authorization as quickly as possible.
Q: Does anyone in your office speak Spanish?
A: Yes, we have someone who speaks Spanish in our office. However, it is best if you can bring an English speaking translator with you to your appointment in case our Spanish speakers are not available.
