He was tired of feeling helpless and alone, and he wanted to get his life back on track. At first, his weekly sessions had a positive impact on his life and psychological well-being. Then he revealed his sexual orientation to his therapist, and the experience went downhill. \u201cShe specialized in marriage and couples counseling,\u201d Campbell says, \u201cand when I told her I was in a same-sex relationship, she seemed distant. This was in New York City, of all places.\u201d<\/p>\n<p>Finding a therapist you like and trust can be challenging for anyone. But members of the queer and trans communities \u2014 a population with a threefold risk for <a href=\"https:\/\/www.nami.org\/Find-Support\/LGBTQ\">anxiety disorders and major depression<\/a>, as well as a disproportionate rate of <a href=\"https:\/\/www.nami.org\/Find-Support\/LGBTQ\">suicidal behavior<\/a> \u2014 face an outsized battle when it comes to obtaining mental healthcare. Not only are LGBTQ folks more likely than their straight, cisgender counterparts to deal with mental illness, they\u2019re also less likely to get clinical help for it. When they do enter treatment, they report <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4936529\/\">low levels of satisfaction <\/a>and high dropout rates.<\/p>\n<p>That\u2019s not to say LGBTQ patients should write off head-shrinking and talking it out. Experts in the field, as well as patients who\u2019ve found therapists they like and trust, say it\u2019s worth holding out for a high-quality professional. But even in 2018, in places as LGBTQ-friendly as New York City, finding the right therapist can take patience and perseverance.<\/p>\n<blockquote><p>\u201cI kissed many frogs before finding my current therapist.\u201d<\/p><\/blockquote>\n<p>In 15 years of on-and-off therapy, Mikki Coleman, 22, who\u2019s trans and nonbinary, has switched therapists more times than they can keep track of. \u201cI can find therapists that are informed on sexual trauma and PTSD,\u201d Coleman says, \u201cbut not on queer or trans identities and experiences. If a therapist can&#8217;t even get my pronouns right, how am I supposed to trust them with the most vulnerable pieces of myself?\u201d<\/p>\n<p>The research on LGBTQ therapy experiences is limited, but Coleman\u2019s complaint jibes with the small number of published studies. <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/labs\/pubmed\/23473079-survey-of-lesbian-gay-bisexual-and-transgender-peoples-experiences-of-mental-health-services-in-ireland\/?from_term=LGBT+mental+health+satisfaction+therapy+&amp;from_pos=1\">In one survey<\/a> on LGBTQ mental healthcare in Ireland, for instance, half of respondents said their therapists lacked knowledge of LGBTQ issues, and nearly as many reported feeling as though practitioners were unresponsive to their needs.<\/p>\n<p>There just aren\u2019t enough therapists with a strong grasp of <a href=\"https:\/\/www.law.columbia.edu\/pt-br\/news\/2017\/06\/kimberle-crenshaw-intersectionality\">intersectionality<\/a>, according to <a href=\"https:\/\/www.vivawellnessnyc.com\/our-practice\/\">Jor-El Caraballo<\/a>, a licensed mental health counselor in Brooklyn. \u201cWhen clinicians don\u2019t understand how someone might experience being gay and an immigrant, or trans and black,\u201d Caraballo says, \u201cI think that this can cause misunderstanding or miscommunication in the therapeutic relationship, leading to early dropout in therapy.\u201d<\/p>\n<p>\u201cEven working at a hospital that advertises itself as LGBTQ-friendly, there is some homophobia and transphobia expressed by providers,\u201d says <a href=\"https:\/\/mobile.twitter.com\/amandawfrey\">social worker Amanda Frey<\/a>, a bisexual woman who\u2019s been both patient and therapist.<\/p>\n<p>Otherwise competent therapists can still be under-informed about issues of sexuality and gender. In a recent session, Frey\u2019s own therapist confessed that, while she\u2019s worked with lesbian, gay and bisexual patients Frey has referred to her, she wouldn\u2019t feel equipped to furnish therapy to a transgender or gender-nonconforming person.<\/p>\n<p>Barriers aside, Frey and Caraballo insist that LGBTQ-friendly therapists are out there. Frey recommends forging on past each awkward encounter and unfulfilling introductory session \u2014 kind of like dating: \u201cI kissed many frogs before finding my current therapist.\u201d<\/p>\n<p>To expedite the search, Frey suggests throwing all your cards on the table right away. \u201cBe straightforward about the things you need from the therapy relationship by stating, \u2018I am a queer woman and I need someone who is LGBTQ-experienced and competent to work through some relationship stuff with me\u2019 or \u2018I\u2019m a trans dude and I need someone who understands poly relationships.\u2019\u201d<\/p>\n<blockquote><p>Sometimes the therapist an LGBTQ patient needs isn\u2019t merely someone who\u2019s worked with the community \u2014 it\u2019s someone who\u2019s also part of it.<\/p><\/blockquote>\n<p>Sometimes the therapist an LGBTQ patient needs isn\u2019t merely someone who\u2019s worked with the community \u2014 it\u2019s someone who\u2019s also part of it. \u201cWhen you&#8217;re trans or queer, the most basic and intrinsic parts of yourself are often what you have to explain in the most depth before you can even get to the actual therapy,\u201d says Coleman. \u201cTo have a therapist concerned about my well-being as it relates to my identity was absolutely revolutionary for me.\u201d<\/p>\n<p>Coleman didn\u2019t connect with any therapist until they found a queer woman with a decade of specialized experience in trans healthcare. \u201cI never had to explain my identity at any point,\u201d says Coleman of the therapist they finally clicked with.<\/p>\n<p>Hannah Rimm, a 25-year-old queer woman, similarly endured years of trial and error before finding her match, a \u201cperfect, helpful, very queer therapist.\u201d<\/p>\n<p>\u201cMy biggest struggle in all of my previous therapists and psychiatrists was feeling completely comfortable to talk about my sexuality and gender expression with somebody who not only was accepting, but who also had experienced questioning sexuality themselves,\u201d says Rimm.<\/p>\n<p>Rimm and her therapist are still going strong after three years, and she\u2019s referred six of her queer friends to them as well.<\/p>\n<p>\u201cIn the case of some marginalized identities, because you see this with people of color seeking out therapists too, it is often important for the patient or client to know that this person really understands,\u201d Frey says. \u201cMost people with marginalized identities have been burnt at least once before in regards to being a mental health service consumer.\u201d<\/p>\n<p>Frey tells patients right away that she\u2019s LGBTQ-friendly. She also makes sure to \u201ctalk the talk\u201d: \u201cI use inclusive language,\u201d she says. \u201cI respect pronouns, and I try my best not to assume.\u201d<\/p>\n<p>She doesn\u2019t go out of her way to advertise her own LGBTQ identity, but she is open to sharing that information. \u201cIf a patient asked me in the context of private practice what I identified as, I wouldn\u2019t have a problem disclosing to them that I\u2019m a bisexual cisgender woman.\u201d<\/p>\n<p>Beyond that, Frey says, she\u2019s careful about the personal details she divulges. \u201cIt can distract from the reason we\u2019re both there: to treat the patient.\u201d<\/p>\n<hr \/>\n<h1 style=\"text-align: center;\">Ready to book a therapy appointment?
