{"id":18368,"date":"2019-10-01T16:10:43","date_gmt":"2019-10-01T21:10:43","guid":{"rendered":"http:\/\/thepapergown.zocdoc.com\/?p=18368"},"modified":"2023-03-03T15:28:48","modified_gmt":"2023-03-03T20:28:48","slug":"these-two-things-set-good-doctors-apart-from-the-rest","status":"publish","type":"post","link":"https:\/\/www.zocdoc.com\/blog\/healthcare-trends\/these-two-things-set-good-doctors-apart-from-the-rest\/","title":{"rendered":"These Two Things Set &#8216;Good&#8217; Doctors Apart"},"content":{"rendered":"<p><span style=\"font-weight: 400;\">As patients, we\u2019ve probably all seen doctors we thought were better and worse at their jobs.<\/span> <span style=\"font-weight: 400;\">I met my favorite doctor in graduate school. During my first appointment with her, she remarked with excitement, \u201cOh, you\u2019re a PhD student \u2014 you\u2019ll get this!\u201d and then took the time to carefully provide a detailed explanation of a procedure she was recommending.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">But what exactly is it that sets apart the \u201cgood doctors\u201d from the ones who leave a less-than-favorable impression? To find out, Stanford psychologists Kari Leibowitz and Alia Crum and I recently collected stories about patients\u2019 healthcare experiences.<\/span> <span style=\"font-weight: 400;\">It turns out what seems to matter most is whether your doctor exhibits the right mix of medical know-how and investment in you as a person.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In a survey, the findings of which were <\/span><a href=\"https:\/\/www.frontiersin.org\/articles\/10.3389\/fpsyt.2019.00475\/full?report=reader\"><span style=\"font-weight: 400;\">recently published<\/span><\/a><span style=\"font-weight: 400;\"> in <\/span><i><span style=\"font-weight: 400;\">Frontiers in Psychiatry<\/span><\/i><span style=\"font-weight: 400;\">, we asked 334 adults in the U.S., from a variety of racial\/ethnic and socioeconomic backgrounds, <\/span><span style=\"font-weight: 400;\">to explain what a good and a bad doctor meant to them. First, we asked survey participants to describe the positive characteristics that their ideal doctor would possess:<\/span> <i><span style=\"font-weight: 400;\">Imagine what a good doctor would be like. What good things would this doctor do? What good qualities would this doctor have? <\/span><\/i><span style=\"font-weight: 400;\">This helped us understand what participants were looking for in a dream doctor, whether or not they&#8217;d ever interacted with one who checked all the boxes.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Then survey participants reflected on their own, real-life healthcare experiences. First, they indicated whether they had ever seen a good doctor. If they said yes, we asked what was good about that doctor. In addition to learning about the platonic ideal of a doctor, we were able to get a sense of what appealed to participants about doctors they\u2019d actually seen and liked.<\/span><\/p>\n<blockquote><p><span style=\"font-weight: 400;\">In our paper, we suggest that provider competence and warmth are the key \u201cingredients\u201d in patient-provider interactions.<\/span><\/p><\/blockquote>\n<p><span style=\"font-weight: 400;\">Their<\/span> <span style=\"font-weight: 400;\">responses revealed a few common themes. Many descriptions \u2014 of both ideal doctors and good IRL doctors \u2014 concerned<\/span> <span style=\"font-weight: 400;\">whether doctors seemed to \u201cget it,\u201d i.e., whether they demonstrate medical proficiency, knowledge and skill. For example, patients depicted the ideal doctor as one who would be \u201cup to date with newer medical studies.\u201d One patient described a good doctor they&#8217;d seen as one who \u201chad a good knowledge of his field.\u201d Others described doctors who were thorough, attentive and had a good work ethic, such as one who \u201clooks at any and all alternatives\u201d and \u201cwould follow up on small concerns.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The other good-doctor descriptions reflected considerations of whether your doctor seems to \u201cget you,\u201d i.e., whether they demonstrate personal engagement, an interest in connecting and a commitment to <\/span>patient care. Patients shared stories about good doctors who \u201chad genuine concern about my well-being\u201d and \u201clistened to what I was saying.\u201d Many of these stories emphasized the importance of being treated as an individual \u2014 for example, doctors who \u201ctreated me as a person, not an illness\u201d and \u201casked me questions about what was going on in my life,\u201d like my doctor who acknowledged that outside the exam room, I had a life as a PhD student.<\/p>\n<p><span style=\"font-weight: 400;\">These examples all fall into the categories that the psychological literature on <\/span><a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S019130851100013X\"><span style=\"font-weight: 400;\">social perception<\/span><\/a><span style=\"font-weight: 400;\"> calls competence. That includes qualities like intelligence, ambition and skill (the \u201cgets it\u201d aspect of medicine), and warmth, meaning qualities like friendliness, empathy and kindness (the \u201cgets me\u201d aspect). In fact, <\/span><a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S1364661306003299\"><span style=\"font-weight: 400;\">research suggests<\/span><\/a><span style=\"font-weight: 400;\"> that these are two key dimensions that we pick up on when we first encounter any new person.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In our paper, we suggest that provider competence and warmth are the key \u201cingredients\u201d in patient-provider interactions. The example I shared of my favorite doctor captures both dimensions. She made it clear that she got me as a patient: She remembered something I had told her about my personal life (that I was a PhD student) and seemed to change her behavior toward me accordingly. By giving me a thorough and clear explanation of a complex medical procedure, she also showed me that she got it<\/span><i><span style=\"font-weight: 400;\">. <\/span><\/i><span style=\"font-weight: 400;\">Because she demonstrated both of these qualities in tandem, I trusted this doctor more than others I\u2019ve seen where those qualities were less apparent. (A close runner-up for my favorite doctor was one who insisted on meeting me in person before I made an appointment for surgery, saying that he likes to get to see who his patients are before conducting an operation on them. Again, this highlights how important it can be to feel like your doctor sees you as an individual rather than just another body in the operating room.)<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Of course, when it comes to displaying competence and warmth to patients, one size doesn\u2019t fit all<\/span><span style=\"font-weight: 400;\">. Providers could communicate both qualities to patients in many ways, and what works for one doctor or patient might not work for others. My doctor taking extra time to explain a procedure was great for me. Some patients might not be interested in so much scientific detail, or might prefer a doctor to realize they have a child sitting in the waiting room and just get to the point.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">As another example, <\/span><span style=\"font-weight: 400;\">touching a patient on the shoulder might feel genuine and help communicate warmth effectively for one provider, whereas for a different one, it might seem like an unnatural gesture and thus make the patient feel less, rather than more, comfortable. Ultimately, what appears to matter for healthcare is that patients perceive providers as both \u201cgetting it\u201d and \u201cgetting me\u201d \u2014 regardless of how they get there.<\/span><\/p>\n<blockquote><p><span style=\"font-weight: 400;\">A <\/span><a href=\"https:\/\/www.nejm.org\/doi\/full\/10.1056\/NEJMp1504023\"><span style=\"font-weight: 400;\">large literature<\/span><\/a><span style=\"font-weight: 400;\"> suggests that when we expect to get better, we actually improve in response.<\/span><\/p><\/blockquote>\n<p><span style=\"font-weight: 400;\">Our research also suggests that not only does exhibiting competence and warmth make patients view doctors favorably, it could also influence the effectiveness of medical care. My colleagues and I ran one <\/span><a href=\"https:\/\/psycnet.apa.org\/record\/2017-10534-001\"><span style=\"font-weight: 400;\">study<\/span><\/a><span style=\"font-weight: 400;\"> in which volunteer patients received treatment for mild, lab-induced allergic skin reactions. Patients had stronger (positive) responses to treatment, we found, when it was administered by providers who displayed high levels of both warmth and competence.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">This finding is supported by a few other recent studies in which patients were more responsive to treatments when providers connected with them socially (see <\/span><a href=\"https:\/\/www.bmj.com\/content\/336\/7651\/999\/\"><span style=\"font-weight: 400;\">here<\/span><\/a><span style=\"font-weight: 400;\">, <\/span><a href=\"https:\/\/www.frontiersin.org\/articles\/10.3389\/fpsyg.2016.00874\/full\"><span style=\"font-weight: 400;\">here<\/span><\/a><span style=\"font-weight: 400;\"> and <\/span><a href=\"https:\/\/academic.oup.com\/ptj\/article\/94\/4\/477\/2735610\"><span style=\"font-weight: 400;\">here<\/span><\/a><span style=\"font-weight: 400;\">). Not only did the positive experience I had with my doctor make the task of managing a health issue far more enjoyable than usual, but it might have increased my confidence that the treatment regimen she chose for me would work well. In turn, my enhanced expectations about treatment might have led me to exhibit more healing after medical care, since a <\/span><a href=\"https:\/\/www.nejm.org\/doi\/full\/10.1056\/NEJMp1504023\"><span style=\"font-weight: 400;\">large literature<\/span><\/a><span style=\"font-weight: 400;\"> suggests that when we expect to get better, we actually improve in response. The placebo effect is the classic demonstration of this phenomenon.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Given these findings, patients might consider whether a healthcare provider seems to embody competence and warmth when choosing where and from whom to get medical care. Finding a doctor with both of these qualities could make trips to the doctor\u2019s office less stressful and more satisfying. And the very same medical treatment might even have more of an impact when it\u2019s administered by a doctor who seems to \u201cget it\u201d and \u201cget you.\u201d\u00a0<\/span><\/p>\n<hr \/>\n<h1 class=\"p1\" style=\"text-align: center;\"><span class=\"s1\">Ready to book a doctor&#8217;s appointment?
