{"id":18677,"date":"2020-04-24T10:49:37","date_gmt":"2020-04-24T15:49:37","guid":{"rendered":"http:\/\/thepapergown.zocdoc.com\/?p=18677"},"modified":"2023-03-06T11:14:49","modified_gmt":"2023-03-06T16:14:49","slug":"we-all-know-inflammation-is-bad-but-what-exactly-is-it","status":"publish","type":"post","link":"https:\/\/www.zocdoc.com\/blog\/guides\/we-all-know-inflammation-is-bad-but-what-exactly-is-it\/","title":{"rendered":"We All Know Inflammation Is Bad, but What Exactly Is It?"},"content":{"rendered":"<p><span style=\"font-weight: 400;\">Nearly 90 percent of Americans who\u2019ve been hospitalized with COVID-19 <\/span><a href=\"https:\/\/www.cdc.gov\/mmwr\/volumes\/69\/wr\/mm6915e3.htm\"><span style=\"font-weight: 400;\">had underlying health condition<\/span><\/a><span style=\"font-weight: 400;\">s, including hypertension, diabetes and chronic lung disease. Most chronic disease is linked in some way to inflammation in the body. But what is inflammation exactly? It might be hard to tell based on recent (pre-coronavirus) headlines. On one hand, a study <\/span><a href=\"https:\/\/www.bostonherald.com\/2020\/02\/17\/veterans-with-gulf-war-syndrome-have-widespread-brain-inflammation-new-study-shows\/\"><span style=\"font-weight: 400;\">showed widespread brain inflammation<\/span><\/a><span style=\"font-weight: 400;\"> in veterans with Gulf War Syndrome. On the other hand, you can find articles touting anti-inflammatory foods as a way to &#8220;melt&#8221; belly fat. <\/span><span style=\"font-weight: 400;\">Are they talking about the same phenomenon?<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In general terms, inflammation is a defense mechanism.<\/span> <span style=\"font-weight: 400;\">You might think of a sore throat or a sprained ankle that\u2019s swollen and tender. Those are examples of <\/span><a href=\"https:\/\/www.nature.com\/subjects\/acute-inflammation\"><span style=\"font-weight: 400;\">acute inflammation<\/span><\/a><span style=\"font-weight: 400;\">, a short-term<\/span> <span style=\"font-weight: 400;\">response to bodily harm with five hallmark symptoms: pain, redness, swelling, loss of function and heat. Essentially, it\u2019s your body\u2019s way of protecting you, by sending white blood cells to injured tissue to begin the healing process.<\/span><\/p>\n<p><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK493173\/\"><span style=\"font-weight: 400;\">Chronic inflammation<\/span><\/a><span style=\"font-weight: 400;\">, on the other hand, is slow-moving and destructive. Instead of using these mechanisms for healing, the body targets healthy tissue, essentially attacking itself.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cAn acute inflammatory response is healthy,\u201d says Donna Arnett, an epidemiologist and the current dean of the University of Kentucky\u2019s College of Public Health. \u201cWhen someone develops an infection, part of the inflammatory response is to turn on our immune system to fight the infection.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">During chronic inflammation, the body remains in a prolonged, low-grade state of emergency, sometimes for years. It often emerges in response to an autoimmune disease or exposure to environmental toxins like air pollution. It\u2019s also been associated with obesity, smoking and a diet rich in saturated fat, trans fats or refined sugar.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">We would do well to understand it better: Chronic inflammation plays a role in almost every major disease. Studies have linked it to cancer, heart disease, diabetes, dementia and mental health disorders. The World Health Organization says chronic inflammatory diseases are the leading cause of death worldwide. <\/span><a href=\"https:\/\/www.cdc.gov\/chronicdisease\/resources\/infographic\/chronic-diseases.htm\"><span style=\"font-weight: 400;\">Six in 10 adults in the US<\/span><\/a><span style=\"font-weight: 400;\"> live with a chronic disease, such as diabetes or chronic lung disease. Four in 10 have two or more chronic diseases.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cIt was only recently that scientists fully appreciate the extent to which inflammation might be a common process underlying risk for several different types of health problems \u2014 everything from depression to anxiety disorders to cardiovascular disease and cancer,\u201d says George Slavich, director of the <\/span><a href=\"https:\/\/www.uclastresslab.org\/\"><span style=\"font-weight: 400;\">UCLA Laboratory for Stress Assessment and Research<\/span><\/a><span style=\"font-weight: 400;\">.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Some studies suggest the roots of chronic inflammation could begin in utero. A recent paper in the journal <\/span><a href=\"https:\/\/www.nature.com\/articles\/s41591-019-0675-0\"><i><span style=\"font-weight: 400;\">Nature Medicine<\/span><\/i><\/a><span style=\"font-weight: 400;\"> examined maternal lifestyle and environmental exposures, including diet, stress, smoking and air pollution, which can influence the way the baby\u2019s immune system is programmed, setting them on a possible course for increased inflammation later in life.\u00a0<\/span><\/p>\n<blockquote><p><span style=\"font-weight: 400;\">What\u2019s especially tricky about chronic inflammation is that you might not know you have it.<\/span><\/p><\/blockquote>\n<p><span style=\"font-weight: 400;\">What\u2019s especially tricky about chronic inflammation is that you might not know you have it. There are no accepted biomarkers. But levels of C-reactive protein in the body increase when inflammation is present, and they can be measured with a simple blood test. Doctors often use CRP tests to detect flare-ups of autoimmune diseases, like lupus and rheumatoid arthritis, but anyone can ask their primary care doctor to perform one. A CRP test can\u2019t reveal the cause of inflammation, though. Arnett points to a <\/span><a href=\"https:\/\/www.ahajournals.org\/doi\/10.1161\/01.ATV.0000248534.30057.b5\"><span style=\"font-weight: 400;\">study<\/span><\/a><span style=\"font-weight: 400;\"> that showed the flu vaccine raised CRP levels acutely, making the test inconclusive. The American Heart Association only recommends the test for <\/span><a href=\"https:\/\/www.mayoclinic.org\/tests-procedures\/c-reactive-protein-test\/about\/pac-20385228?page=0&amp;citems=10\"><span style=\"font-weight: 400;\">patients with an increased risk for having a heart attack<\/span><\/a><span style=\"font-weight: 400;\">.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cWe know that autoimmune disease is associated with increased heart disease risk,\u201d says Arnett, an AHA past president. \u201cHowever, a very <\/span><a href=\"https:\/\/www.nejm.org\/doi\/pdf\/10.1056\/NEJMoa1809798\"><span style=\"font-weight: 400;\">large randomized clinical trial<\/span><\/a><span style=\"font-weight: 400;\"> using a drug to reduce inflammation did not find benefit for reducing CRP or reducing the risk of cardiovascular events.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">While Slavich doesn\u2019t see any harm in doing the test (which can <\/span><a href=\"https:\/\/www.health.harvard.edu\/heart-health\/c-reactive-protein-test-to-screen-for-heart-disease\"><span style=\"font-weight: 400;\">cost around $12 to\u00a0 $16<\/span><\/a><span style=\"font-weight: 400;\">) if you\u2019re curious about your CRP levels, he says chronic inflammation may also be managed through lifestyle changes \u2014 the same habits recommended for general health: Don\u2019t smoke, exercise regularly and get a good night\u2019s sleep.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Diet matters too, as certain foods influence chronic inflammation for better or worse. Research has shown that trans fatty acids and high salt intake lead to increased inflammation. Refined sugars and ultra-processed foods can contribute to the activation of inflammatory genes. On the other hand, foods rich in omega-3 fatty acids, like mackerel, salmon and chia seeds, have shown inflammation-reducing effects; leafy greens and fruits are high in inflammation-fighting <\/span><span style=\"font-weight: 400;\">antioxidants and polyphenols. Many doctors also recommend a Mediterranean diet as an overall anti-inflammatory diet. Coffee makes the cut too, <\/span><a href=\"https:\/\/www.health.harvard.edu\/staying-healthy\/foods-that-fight-inflammation\"><span style=\"font-weight: 400;\">according to Harvard researchers<\/span><\/a><span style=\"font-weight: 400;\">.<\/span><span style=\"font-weight: 400;\">\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">But if a person can make only one lifestyle change to reduce inflammation and improve immune health, according to Slavich, it should be cutting down on stress.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cUnlike genetic factors, which are impossible to change, and access to healthy foods, which can be difficult to change, the ability to change how we perceive the world lies in the hands of all of us,\u201d he says. \u201cStress is the silent killer that everyone knows about but few tackle.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Arnett agrees, and points to yoga as an effective stress-reducer.<\/span><span style=\"font-weight: 400;\"> Stress has been linked to a <\/span><a href=\"https:\/\/www.sciencedaily.com\/releases\/2018\/05\/180529132010.htm\"><span style=\"font-weight: 400;\">shorter lifespan in mice<\/span><\/a><span style=\"font-weight: 400;\">. Research also suggests that certain psychological stressors in the workplace disrupt inflammation-reducing hormones.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">On a brighter note, we are inching toward more actionable data. In a study published in February in the journal <\/span><a href=\"https:\/\/www.cell.com\/cell-metabolism\/fulltext\/S1550-4131(20)30009-7\"><i><span style=\"font-weight: 400;\">Cell Metabolism<\/span><\/i><\/a><span style=\"font-weight: 400;\">, researchers at the University of California, Berkeley, identified what they describe as a \u201cmolecular switch.\u201d It\u2019s a collection of immune system proteins that can be turned off to stop a hyperinflammatory response. Overactivation of this particular collection of proteins has been linked to cancer, multiple sclerosis and dementia. Developing future drugs that target this switch has potential to treat or even prevent some of these conditions, <\/span><a href=\"https:\/\/news.berkeley.edu\/2020\/02\/06\/molecular-switch-reverses-chronic-inflammation-and-aging\/\"><span style=\"font-weight: 400;\">researchers suggest<\/span><\/a><span style=\"font-weight: 400;\">.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cIt&#8217;s really not enough to just know that inflammation is bad,\u201d says Slavich. \u201cWe need to actually begin changing the factors that increase inflammation and damage health.
