{"id":18810,"date":"2020-08-05T15:15:24","date_gmt":"2020-08-05T20:15:24","guid":{"rendered":"http:\/\/thepapergown.zocdoc.com\/?p=18810"},"modified":"2023-03-06T09:51:02","modified_gmt":"2023-03-06T14:51:02","slug":"its-even-more-important-to-get-a-flu-shot-this-year","status":"publish","type":"post","link":"https:\/\/www.zocdoc.com\/blog\/healthcare-trends\/its-even-more-important-to-get-a-flu-shot-this-year\/","title":{"rendered":"Getting a Flu Shot Is More Important Than Ever This Year"},"content":{"rendered":"<p><span style=\"font-weight: 400;\">In recent months, like a lot of people, I\u2019ve pushed off preventive care \u2014 or done it virtually, when possible \u2014 because I&#8217;m scared of getting COVID-19. <\/span><span style=\"font-weight: 400;\">Typically, in-person preventive care is how I get immunized. During some routine visit, a doctor or nurse will ask if I&#8217;ve gotten my flu shot yet. Then they&#8217;ll offer to administer one, and I&#8217;ll say, &#8220;Sure, go for it,&#8221; and roll up my sleeve. Without a built-in opportunity to get the flu vaccine, it&#8217;s tempting to skip it this year.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Many Americans don\u2019t even entertain the idea of getting a flu shot, <a href=\"https:\/\/www.norc.org\/NewsEventsPublications\/PressReleases\/Pages\/37-of-americans-do-not-plan-to-get-a-flu-shot-this-season.aspx\">research suggests<\/a>. In a January poll from the American Academy of Family Physicians, <\/span><a href=\"https:\/\/www.aafp.org\/news\/health-of-the-public\/20200122flusurvey.html\"><span style=\"font-weight: 400;\">one-third of respondents<\/span><\/a><span style=\"font-weight: 400;\"> said they hadn\u2019t gotten one and didn\u2019t plan to.<\/span><b> <\/b><span style=\"font-weight: 400;\">Millennials were the age group least likely to get vaccinated, as well as least informed about the health risks of the flu (86 percent got at least one flu fact wrong). Millennials also had more excuses for not getting vaccinated than older adults; they were twice as likely as boomers to say they forgot or didn\u2019t have time.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">My personal excuse: I live in Florida, a current COVID hotspot, so I\u2019m following disease-prevention guidelines to the letter and then some. I\u2019m not traveling. I work from home, and so does the only other person I live with. With such a small chance of getting or passing along the flu, is it worth venturing out for a flu shot?<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Yes, yes it is.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Last year,<\/span> <a href=\"https:\/\/www.cdc.gov\/flu\/fluvaxview\/coverage-1819estimates.htm\"><span style=\"font-weight: 400;\">45 percent<\/span><\/a> <span style=\"font-weight: 400;\">of American adults got a flu shot, according to the Centers for Disease Control and Prevention. This tepid figure is nowhere near the rate we\u2019d need to reach <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/22414740\/\"><span style=\"font-weight: 400;\">herd immunity<\/span><\/a><span style=\"font-weight: 400;\"> (<\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC6008254\/\"><span style=\"font-weight: 400;\">70 to 80 <\/span><\/a><span style=\"font-weight: 400;\">percent), but <\/span><a href=\"https:\/\/www.cdc.gov\/flu\/fluvaxview\/coverage-1718estimates.htm\"><span style=\"font-weight: 400;\">according to CDC <\/span><\/a><span style=\"font-weight: 400;\">data, <\/span><span style=\"font-weight: 400;\">it is higher than annual adult vaccination rates over the last decade<\/span>. <span style=\"font-weight: 400;\">Public health experts say vaccination is more important than ever this year. Still, they fear too few people are hearing the message.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cWe\u2019re <\/span><span style=\"font-weight: 400;\">hopeful that the more severe the pandemic might be, that people will do everything they can to decrease their risk,\u201d says Dr. Benjamin D. Singer, a pulmonologist and assistant professor at Northwestern University Feinberg School of Medicine. \u201cHow to convince them is a different question.\u201d Singer recently wrote <\/span><a href=\"https:\/\/advances.sciencemag.org\/content\/early\/2020\/05\/28\/sciadv.abd0086\"><span style=\"font-weight: 400;\">an editorial<\/span><\/a><span style=\"font-weight: 400;\"> in the journal <\/span><i><span style=\"font-weight: 400;\">Science Advances<\/span><\/i><span style=\"font-weight: 400;\"> about how the spread of COVID-19 could complicate healthcare during the 2020-2021 flu season. \u201cPeople always ask about the best way to boost the immune system,\u201d he says, \u201cand the No. 1 way you do that is with a vaccine.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Officially, the <\/span><a href=\"https:\/\/www.cdc.gov\/flu\/weekly\/index.htm\"><span style=\"font-weight: 400;\">2019-2020 flu season<\/span><\/a><span style=\"font-weight: 400;\"> concluded at the end of July, <\/span><span style=\"font-weight: 400;\">but peak<\/span> <span style=\"font-weight: 400;\">flu season is November through February<\/span>.<span style=\"font-weight: 400;\"> Since<\/span> <span style=\"font-weight: 400;\">COVID-19 diagnoses started ramping up in the US as peak flu season was winding down, we haven&#8217;t had to endure a full season with both viruses circulating at once. While the flu vaccine won\u2019t protect people from getting COVID-19, one of the primary ways flu shots \u201cwork\u201d is by preventing infections severe enough to warrant a hospital visit. Flu hospitalizations also increase the burden on hospitals already overwhelmed with COVID-19 cases.<\/span><\/p>\n<blockquote><p>\u201cIt\u2019s hard enough to get people vaccinated even when there isn\u2019t a pandemic.&#8221;<\/p><\/blockquote>\n<p><span style=\"font-weight: 400;\">Optimists predict that measures to help contain COVD-19, such as increased social distancing, vigilant hand-washing and greater use of hand sanitizer, could reduce the spread of the flu in the US this year. <\/span><a href=\"https:\/\/www1.health.gov.au\/internet\/main\/publishing.nsf\/Content\/C212D5131C8E4EDACA25858B0024608B\/$File\/flu-05-2020.pdf\"><span style=\"font-weight: 400;\">Australia\u2019s rate of flu infection plummeted<\/span><\/a><span style=\"font-weight: 400;\"> during its season. In April, the country saw 229 flu cases, compared to 18,000 in April 2019. Health officials speculate that stepped-up disease prevention awareness, along with school closures, might have contributed to the decrease. But they also said vaccination rates were much higher than usual: More than twice as many Australians got the flu shot in 2020 <\/span><a href=\"https:\/\/www.health.gov.au\/ministers\/the-hon-greg-hunt-mp\/media\/record-flu-vaccines-in-2020-to-protect-australians\"><span style=\"font-weight: 400;\">than 2018<\/span><\/a><span style=\"font-weight: 400;\">.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Evidence also suggests the flu season may have ended abruptly in Japan when people started taking the precautions against COVID-19 seriously, says Stephen S. Morse, professor of epidemiology at Columbia University. Whether Americans will follow suit, however, remains to be seen.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">For one thing, <\/span><a href=\"https:\/\/link.springer.com\/article\/10.1007\/s40272-020-00385-4\"><span style=\"font-weight: 400;\">vaccine hesitancy<\/span><\/a><span style=\"font-weight: 400;\"> appears to be on the upswing, and some of the usual avenues for people to get the flu shot have dried up, making vaccination less convenient. Health experts worry that if we have a bad flu season, dueling pandemics could overwhelm healthcare workers and facilities. But even if the coming flu season doesn\u2019t earn <\/span><a href=\"https:\/\/www.cdc.gov\/flu\/pandemic-resources\/basics\/about.html\"><span style=\"font-weight: 400;\">pandemic status<\/span><\/a><span style=\"font-weight: 400;\">, it\u2019s likely to complicate the fight against COVID-19.<\/span><\/p>\n<p>\u201cIt\u2019s hard enough to get people vaccinated even when there isn\u2019t a pandemic,\u201d says<a href=\"https:\/\/www.orlandohealth.com\/physician-finder\/jean-siri-moorjani-md#\/overview?hcmacid=a0bi000000IJI9DAAX\"> Dr. Jean S. Moorjani<\/a>, a pediatrician in Orlando, Florida. \u201cThere\u2019s so much hesitancy and mistrust, I feel like we\u2019re bracing ourselves for the next few months.\u201d Patients (or patients\u2019 parents) are often skeptical that the flu shot is worth getting, Moorjani says, and will ask, \u201cIs it really that effective? I get sick anyway.\u201d<\/p>\n<p><span style=\"font-weight: 400;\">Poor flu vaccine compliance in the US may fall even further with COVID-related fears and anti-vaccine myths, says Dr. Don L. Goldenberg, a rheumatologist, professor emeritus of medicine at Tufts University School of Medicine and the author of <\/span><i><span style=\"font-weight: 400;\">How the COVID-19 Pandemic Is Affecting You and Your Healthcare<\/span><\/i><span style=\"font-weight: 400;\">.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cThat would be disastrous for a number of reasons,\u201d Goldenberg says. \u201cNo. 1, COVID-19 and influenza both cause lung infection\/inflammation, and if [these] spread at the same time, [that] likely would overwhelm hospitals and ICUs and lead to severe shortage of critical items, such as ventilators.\u201d<\/span><\/p>\n<blockquote><p><span style=\"font-weight: 400;\">In case you didn\u2019t catch that: It is possible to have the flu and COVID-19 at the same time.<\/span><\/p><\/blockquote>\n<p><span style=\"font-weight: 400;\">In addition, both COVID and the flu carry the same increased risk of infection and death, particularly for older patients and racial minorities, he says: \u201cThese groups will be more at risk for co-infection with both viruses, as well as for poor outcomes from either infection.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In case you didn\u2019t catch that: It is possible to have the flu and COVID-19 at the same time, which can make diagnosis and treatment for both more difficult.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cHealthcare providers may have difficulty determining the correct diagnosis, especially if testing for COVID-19 continues to be inadequate,\u201d Goldenberg says. \u201cThis may result in inappropriate quarantine and treatment recommendations.\u201d\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Because researchers are still learning about COVID-19, they can\u2019t yet say whether having both viruses at the same time increases your chances of dying.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cLogically, with two severe viruses, I think the risk for dying would be higher, but it\u2019s unclear,\u201d Singer says. \u201cThe only way to know is to do a test that comes back very quickly. We have to know quickly who has it so they can isolate, and it also helps with contact tracing.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">There are psychological ramifications of severe illness as well. Even pre-COVID-19, evidence suggested that surviving a severe flu infection can be traumatic.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cThere are a lot of long-term, well-known effects from being in the ICU, because it\u2019s a stressful environment,\u201d Singer says. \u201cThere are also neurological muscle problems and profound weakness after a severe course of the flu. <\/span><span style=\"font-weight: 400;\">It\u2019s not trivial.<\/span><b>\u201d<\/b><\/p>\n<p><span style=\"font-weight: 400;\">Another complication that doctors can still only guess at is whether recovering from COVID-19 might make you more susceptible to contracting the flu, and vice versa.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cUnfortunately, the risk for severe flu illness in people recovering from COVID-19 infection is largely unknown,\u201d says Dr. Joi Lucas, a pediatric pulmonologist in central Florida.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Conversely, she adds, people should vaccinate against the flu to protect themselves from severe respiratory complications that make them even more susceptible to COVID-19.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cIllness with the flu alone is concerning, so the commingling of COVID-19 and the flu may be debilitating,\u201d Lucas continues. \u201cStudies have shown people with COVID-19 have persistent chest X-ray abnormalities, <\/span><span style=\"font-weight: 400;\">and the long-term effects of infection with the virus are unknown.\u201d\u00a0<\/span><\/p>\n<blockquote><p><span style=\"font-weight: 400;\">&#8220;A great deal of thought and effort has gone into how to return to normal care, including immunizations, as safely as possible.&#8221;<\/span><\/p><\/blockquote>\n<p><span style=\"font-weight: 400;\">The CDC has issued <\/span><a href=\"https:\/\/www.cdc.gov\/vaccines\/pandemic-guidance\/index.html\"><span style=\"font-weight: 400;\">COVID-19-specific flu vaccination guidelines<\/span><\/a><span style=\"font-weight: 400;\"> for the upcoming season, noting that people who suspect they might have COVID-19 should hold off getting a flu shot to avoid putting healthcare workers and other patients at risk. The agency also recommends that healthcare providers screen for symptoms before patients come in for their appointments; limit and monitor entrances and exits to decrease patients\u2019 exposure to one another; and implement social distancing guidelines such as floor markers, roped-off areas and electronic check-ins whenever possible.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">CDC officials also say that they\u2019re working with healthcare providers to develop innovative vaccination distribution measures to keep vaccine administrators and patients safer, such as drive-through clinics, mobile clinics and curbside vaccinations.\u00a0<\/span><span style=\"font-weight: 400;\">But if al fresco flu shots aren\u2019t available in your area, don\u2019t despair. Flu shot administrators at doctors\u2019 offices and pharmacies should be familiar with the proper protocols to administer this year\u2019s flu shot safely.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cFrom my experience with helping to develop return-to-work plans at my own university\u2019s medical center, a great deal of thought and effort has gone into how to return to normal care, including immunizations, as safely as possible,\u201d Morse says.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">People should be more concerned about the steps they take to get to vaccination sites than the sites themselves, he says.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cBy time you get there, that\u2019s probably the safest place you\u2019ll be outside your own home,\u201d Morse says. \u201cPlaces that are giving these immunizations have been taking careful precautions, from extra cleaning to increasing ventilation, social distancing and keeping patients separated if applicable. To the best of my knowledge, no one wants to be known as the place where someone went for a flu shot and got COVID instead.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The best time to get a flu shot is September or October, <\/span><a href=\"https:\/\/news.un.org\/en\/story\/2020\/06\/1066372\"><span style=\"font-weight: 400;\">according to the United Nations<\/span><\/a><span style=\"font-weight: 400;\"> and the CDC. It\u2019s possible, <\/span><a href=\"https:\/\/www.sciencemag.org\/news\/2019\/04\/how-long-do-vaccines-last-surprising-answers-may-help-protect-people-longer%20https:\/\/news.llu.edu\/patient-care\/how-long-do-flu-shots-last\"><span style=\"font-weight: 400;\">research suggests<\/span><\/a><span style=\"font-weight: 400;\">, that getting one too early could mean not having enough flu-fighting antibodies in your system through February, when the virus\u2019s spread is <\/span><a href=\"https:\/\/www.cdc.gov\/flu\/about\/season\/flu-season.htm\"><span style=\"font-weight: 400;\">typically at its peak<\/span><\/a><span style=\"font-weight: 400;\">.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">If you\u2019re able to work from home right now, it might be tempting to rationalize that your risk of getting the flu is small enough to warrant skipping a flu shot this year. But health experts hope everyone who should get a flu shot (read: like me) will.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cIt\u2019s hard to know over the course of months who you\u2019re going to be exposed to, even if day to day, you\u2019re working from home,\u201d Singer says. \u201cYou never know when you might have to make an unexpected trip. And there\u2019s a very small risk in going out to get the vaccine compared with the potential benefit from being vaccinated.\u201d<\/span><\/p>\n<hr class=\"squiggle\" \/>\n<p><span style=\"font-weight: 400;\">A couple weeks ago, I ran an informal Facebook poll about the flu shot and got a range of answers from 32 friends and acquaintances. Those not planning to get vaccinated cited reasons including needle fears, doubts over the flu shot working and not needing one because they\u2019re vigilant about hand-washing. Few seemed aware of experts\u2019 contention that flu vaccination is particularly important during the pandemic.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">But<\/span> <span style=\"font-weight: 400;\">most people said they will get a flu shot, some for the first time in many years on account of the pandemic \u2014 which Morse might find encouraging right now.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cI\u2019m heartbroken,\u201d he said in a recent email regarding the spread of COVID-19. \u201cI\u2019ve devoted the last three decades of my career to trying to prevent something like this from happening.\u201d<\/span><\/p>\n<hr \/>\n<h1 class=\"p2\" style=\"text-align: center;\"><span class=\"s1\">Ready to book a doctor&#8217;s appointment?
