{"id":18923,"date":"2020-11-09T15:40:36","date_gmt":"2020-11-09T20:40:36","guid":{"rendered":"http:\/\/thepapergown.zocdoc.com\/?p=18923"},"modified":"2026-02-05T04:48:05","modified_gmt":"2026-02-05T09:48:05","slug":"does-getting-an-iud-hurt","status":"publish","type":"post","link":"https:\/\/www.zocdoc.com\/blog\/guides\/does-getting-an-iud-hurt\/","title":{"rendered":"Does Getting an IUD Hurt?"},"content":{"rendered":"<p><span style=\"font-weight: 400;\">Like many women of reproductive age, my friends and I frequently discuss contraception: what we\u2019re using, what we\u2019ve tried, what we\u2019re curious about. Without a doubt, the most polarizing form of birth control among my cadre of confidantes is the intrauterine device, or IUD.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">An IUD is a small T-shaped apparatus that, when inserted into the uterus, <\/span><a href=\"https:\/\/www.zocdoc.com\/blog\/guides\/what-to-know-before-getting-an-iud\/\"><span style=\"font-weight: 400;\">releases hormones or (in non-hormonal IUDs) copper<\/span><\/a><span style=\"font-weight: 400;\"> to prevent sperm from fertilizing eggs. A <\/span><a href=\"https:\/\/www.contraceptionjournal.org\/article\/S0010-7824%2815%2900072-4\/pdf\"><span style=\"font-weight: 400;\">favorite among gynecologists<\/span><\/a><span style=\"font-weight: 400;\">, IUDs are a long-lasting, low-maintenance form of contraception. Once placed, they last between three and 15 years.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Despite these upsides, one aspect of the IUD experience tends to garner negative attention: the insertion process. Is getting an IUD placed as painful as rumor would have it?<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Answers vary \u2014 a lot. Anecdotally, a former roommate of mine said getting her IUD was memorably uncomfortable and that she bled continuously for weeks afterward. On the other hand, my current roommate only<\/span> <span style=\"font-weight: 400;\">experienced three short cramps during the procedure and then went about her day. Research on IUD insertion is similarly inconsistent. In a <\/span><a href=\"https:\/\/www.tandfonline.com\/doi\/abs\/10.3109\/13625187.2011.558222?journalCode=iejc20\"><span style=\"font-weight: 400;\">2011 study<\/span><\/a><span style=\"font-weight: 400;\"> of 224 women who had never given birth, most participants rated IUD insertion as moderately painful (72 percent) or severely painful (17 percent). Only 9 percent reported feeling no pain. And a<\/span><span style=\"font-weight: 400;\">\u00a0<\/span><a href=\"https:\/\/www.contraceptionjournal.org\/article\/S0010-7824%2813%2900598-2\/fulltext\"><span style=\"font-weight: 400;\">2013 study<\/span><\/a><span style=\"font-weight: 400;\"> found medical providers often underestimate how painful IUD insertion is for patients. <\/span><span style=\"font-weight: 400;\">A 2015<\/span> <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC5683140\/\"><span style=\"font-weight: 400;\">study<\/span><\/a><span style=\"font-weight: 400;\">, however, concluded that IUD insertion was less painful than patients anticipated.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">So why is getting an IUD harrowing for some patients, and no big deal for others? Can you do anything to reduce your chances of joining the \u201cseverely painful\u201d group? With help from experts, we put together a primer on IUD insertion pain.<\/span><\/p>\n<hr \/>\n<h3>Consider your medical history<\/h3>\n<p><span style=\"font-weight: 400;\">There are three different points during IUD insertion that might be painful, says <\/span><a href=\"https:\/\/njms-web.njms.rutgers.edu\/profile\/myProfile.php?mbmid=brandikm\"><span style=\"font-weight: 400;\">Dr. Kristyn Brandi<\/span><\/a><span style=\"font-weight: 400;\">, an ob-gyn and assistant professor at Rutgers New Jersey Medical School. The first is when the doctor inserts the speculum, just like they would during a pap smear. The second point is when the doctor pushes the IUD through the cervix, the lower, narrow part of the uterus. Finally, you might feel pain at the moment the IUD reaches the top of the uterus (its final destination). This discomfort can feel like <\/span><a href=\"https:\/\/www.cosmopolitan.com\/sex-love\/news\/a52561\/what-getting-an-iud-really-feels-like-according-to-women\/\"><span style=\"font-weight: 400;\">pinching <\/span><\/a><a href=\"https:\/\/www.self.com\/story\/17-women-describe-what-it-actually-feels-like-to-get-an-iud\"><span style=\"font-weight: 400;\">inside the uterus<\/span><\/a><span style=\"font-weight: 400;\">, and may result in cramps that last anywhere from a few seconds to a few hours.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Thinking about your last pap smear can give you an indication of how you might tolerate IUD insertion. \u201c<\/span><span style=\"font-weight: 400;\">Imagine a pap smear, but it [takes] maybe five to 10 minutes longer to place an IUD,\u201d Brandi says. \u201cIf the pap smear experience is a really uncomfortable experience for people, and that happens for a lot of different reasons, then placing an IUD probably is not going to be a good experience.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">If you\u2019ve never given birth vaginally, and haven\u2019t had an abortion or any other procedure on your cervix, it can be particularly uncomfortable when the IUD is placed through the cervix, says Dr. <\/span><a href=\"https:\/\/twitter.com\/hanaobgyn\"><span style=\"font-weight: 400;\">Hana Reissner<\/span><\/a><span style=\"font-weight: 400;\">, the Complex Family Planning fellow at the American College Of Obstetricians and Gynecologists. \u201cWhen you\u2019ve had a vaginal delivery before, your cervix has opened,\u201d she says. \u201cWhile we don\u2019t have the exact evidence for why discomfort may be different, when someone\u2019s cervix has opened before, it\u2019s likely easier to place the IUD.\u201d Some good news if you already have an IUD: Replacing it may very well be a smooth experience, Brandi says.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">For patients who haven\u2019t had a vaginal birth, your doctor can potentially give you medication to help open your cervix a little, but it probably won\u2019t help that much. \u201cThat medication is pretty crampy,\u201d Brandi says. \u201cIt\u2019s one of the medications we give to start labor. Even though it may help open the cervix, it\u2019s still going to cause pain.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">While IUDs vary in size depending on the amount of hormone in the device (we\u2019re talking millimeters here), Brandi says a larger IUD won\u2019t hurt more to place.<\/span><\/p>\n<p><a href=\"https:\/\/www.zocdoc.com\/obgyns?utm_source=pgbanners&amp;utm_medium=papergown\"><img loading=\"lazy\" decoding=\"async\" class=\"aligncenter wp-image-18931\" src=\"\" alt=\"\" width=\"726\" height=\"154\" \/><\/a><\/p>\n<h3>Don\u2019t psych yourself out<\/h3>\n<p><span style=\"font-weight: 400;\">More often than not, fearing an uncomfortable IUD-insertion experience will result in, well, a more uncomfortable experience, Reissner says. A <\/span><a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S1083318819303067\"><span style=\"font-weight: 400;\">study from this year<\/span><\/a><span style=\"font-weight: 400;\"> showed that people who anticipated more pain ended up having more pain, compared to those who expected insertion to hurt less. Black women tended to report higher levels of anticipated pain than women of other races, the study found.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Nervous jitters are normal before any medical procedure, which is why you should feel empowered to ask your ob-gyn any questions you have before they begin placing your IUD. \u201c<\/span><span style=\"font-weight: 400;\">Developing a plan for what the communication style is like during placement might help ease some of those nerves,\u201d Reissner says. <\/span><span style=\"font-weight: 400;\">Whether you prefer a step-by-step explanation of the procedure as it\u2019s happening, or you\u2019re more of a \u201cjust tell me when it\u2019s over\u201d type of patient, touching base with your doctor beforehand will help assuage your trepidations.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">If you have a diagnosed anxiety disorder, Brandi recommends taking any prescribed daily anti-anxiety medication before your appointment. For her own patients who are not prescribed anti-anxiety medications, Brandi sometimes prescribes a one-time dose if she thinks it would be helpful for the patient.<\/span><\/p>\n<h3>What you can do about potential pain<\/h3>\n<p><span style=\"font-weight: 400;\">While IUD horror stories tend to monopolize the conversation, most patients get through insertion with minimal discomfort, Reissner says. But if you\u2019re exceptionally worried about pain, ask your doctor if a paracervical block \u2014 a local anesthetic injected into your cervix \u2014 is a good numbing option for you, Brandi says. Although the injection itself can hurt, <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC6438819\/\"><span style=\"font-weight: 400;\">recent research<\/span><\/a><span style=\"font-weight: 400;\"> shows that a paracervical block minimizes discomfort during IUD placement. For patients who have a history of sexual assault or conditions like vulvodynia (chronic pain in the vulva), doctors might administer general anesthesia ahead of IUD placement, Brandi says.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Another way to potentially minimize pain is to schedule your IUD insertion during your period, Brandi says. \u201cW<\/span><span style=\"font-weight: 400;\">hat happens is your cervix opens a little bit to help pass [the] bleeding,\u201d she says, \u201cso sometimes that little bit of opening and the lubrication of the blood can make the IUD go in a lot easier.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">One of the best painkillers of all, a <\/span><a href=\"https:\/\/www.sciencedirect.com\/science\/article\/abs\/pii\/S0301211520305960\"><span style=\"font-weight: 400;\">2020 study shows<\/span><\/a><span style=\"font-weight: 400;\">, is talking. \u201c<\/span><span style=\"font-weight: 400;\">I joke [that it\u2019s] called vocal local,\u201d Brandi says. Ask your doctor to chit-chat with you throughout the entire procedure to get your mind off the task at hand. <\/span><span style=\"font-weight: 400;\">But make sure to maintain bodily awareness, and to heed any signals your body sends you. Extreme pain can indicate that an IUD was displaced, meaning it could\u2019ve <\/span><a href=\"https:\/\/www.self.com\/story\/17-women-describe-what-it-actually-feels-like-to-get-an-iud\"><span style=\"font-weight: 400;\">embedded in the sides of your uterus<\/span><\/a><span style=\"font-weight: 400;\">, fallen into the cervix, or has perforated the uterine wall and is in your abdomen, Brandi explains. So keep an open dialogue with your ob-gyn and don\u2019t be afraid to speak up.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In the end, if the thought of IUD insertion makes you skittish, you shouldn\u2019t feel pressured to use one. IUDs have their advantages, <\/span><a href=\"https:\/\/qz.com\/1730175\/birth-control-usage-stats-show-the-pill-losing-ground-to-the-iud\/\"><span style=\"font-weight: 400;\">and they\u2019ve surged in popularity in recent years.<\/span><\/a><span style=\"font-weight: 400;\"> But there are other safe, effective methods of birth control. (Shots, patches, rings \u2014 the list goes on.) Talk to your doctor about your concerns and your personal history.
