PruittHealth, which operates nursing homes and assisted living facilities throughout the Southeast, is trying to incentivize vaccination among employees who might otherwise opt out. \u201cIf that doesn\u2019t get you in line,\u201d said Governor Brian Kemp of Georgia <\/span><a href=\"https:\/\/www.youtube.com\/watch?v=CpTbLjkL2Ko#t=16m40s\"><span style=\"font-weight: 400;\">in a news conference<\/span><\/a><span style=\"font-weight: 400;\">,\u00a0I don\u2019t know what will.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Free breakfast might persuade some vaccine-reluctant people to take the plunge, but according to behavioral scientists, it might be more effective for Kemp to roll up his own sleeve and get the vaccine on live TV. <\/span>Having a community leader model desired behavior is <span style=\"font-weight: 400;\">one of several evidence-based strategies that experts recommend using to overcome vaccine hesitancy.<\/span><\/p>\n<hr class=\"squiggle\" \/>\n<p><span style=\"font-weight: 400;\">To gain control over the pandemic and return to something resembling pre-COVID life, public health leaders emphasize the importance of reaching herd immunity. The safest, fastest and most reliable path to herd immunity is through <\/span><a href=\"https:\/\/www.cdc.gov\/coronavirus\/2019-ncov\/vaccines\/vaccine-benefits.html?CDC_AA_refVal=https%3A%2F%2Fwww.cdc.gov%2Fcoronavirus%2F2019-ncov%2Fvaccines%2Fabout-vaccines%2Fvaccine-benefits.html\"><span style=\"font-weight: 400;\">widespread vaccination<\/span><\/a><span style=\"font-weight: 400;\">. Right now, the most immediate barrier to getting the majority of Americans inoculated is a low supply of vaccines. But even after vaccine production ramps up, and distribution improves, we may still face another hurdle: too many people saying no. While many people are anxiously awaiting V-day \u2014 in some cases traversing cities in hopes of lucking into leftover doses \u2014 not everyone\u2019s desperate to get their shot(s). In fact, vaccine resistance appears to be fairly common.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">A national <\/span><a href=\"https:\/\/www.kff.org\/coronavirus-covid-19\/report\/kff-covid-19-vaccine-monitor-december-2020\/\"><span style=\"font-weight: 400;\">survey<\/span><\/a><span style=\"font-weight: 400;\"> conducted in December by the Kaiser Family Foundation found 27 percent of the general public to be vaccine-hesitant, meaning people who say they\u2019re unlikely to get the COVID vaccine even if it\u2019s free and scientists say it\u2019s safe. Thirty-five percent of <\/span><a href=\"https:\/\/www.kff.org\/coronavirus-covid-19\/perspective\/the-challenge-of-vaccine-hesitancy-in-rural-america\/\"><span style=\"font-weight: 400;\">rural Americans<\/span><\/a><span style=\"font-weight: 400;\"> said they either wouldn&#8217;t or probably wouldn&#8217;t get the vaccine, compared to 26 percent of urban residents. Even among healthcare workers surveyed, 29 percent said they probably wouldn\u2019t get vaccinated. The top three vaccine concerns expressed by respondents were side effects, government mistrust and the vaccine being so new. While it&#8217;s possible that vaccine attitudes have shifted in the last month, the survey results are supported by reports of healthcare workers across the country <\/span><a href=\"https:\/\/www.forbes.com\/sites\/tommybeer\/2021\/01\/02\/large-numbers-of-health-care-and-frontline-workers-are-refusing-covid-19-vaccine\/?sh=5a833c0d3c96\"><span style=\"font-weight: 400;\">refusing vaccination<\/span><\/a><span style=\"font-weight: 400;\">.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cVaccine hesitancy is a continuum,\u201d says Gretchen Chapman, professor of psychology at Carnegie Mellon University where she studies decision-making. Many people who are hesitant to get the COVID vaccine aren\u2019t necessarily opposed to vaccinations in general. \u201c[They] may have some concerns or don\u2019t want to be the first in line, which could be viewed as completely reasonable given how new this vaccine is.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">She likens that form of caution to waiting to buy a just-released smartphone or embrace novel technology, the way many people did when ride-sharing first emerged. \u201cThat strategy of hanging back and seeing how it works for others before you join on average is a fairly good strategy to follow in a lot of parts of life,\u201d she says. \u201cIt just so happens that in this particular situation it\u2019s counterproductive from a public health standpoint. It\u2019s causing a problem.\u201d<\/span><\/p>\n<h2>How to talk to the vaccine-hesitant<\/h2>\n<p><span style=\"font-weight: 400;\">How can we persuade people who want to wait and see, not to mention those who\u2019d rather pass on vaccination altogether, to get in line with the rest of us? <\/span><span style=\"font-weight: 400;\">Experts say that successful efforts to combat vaccine resistance typically employ a few evidence-based strategies, and that the same guidance applies to everything from national public health campaigns to dinnertime debates with skeptical relatives.<\/span><\/p>\n<p><b><i>Drive home facts with thoughtful, consistent messaging\u00a0<\/i><\/b><\/p>\n<p><span style=\"font-weight: 400;\">When it comes to the safety and efficacy of the COVID vaccines (Pfizer and Moderna, specifically), the <\/span><a href=\"https:\/\/www.sciencemag.org\/news\/2020\/11\/absolutely-remarkable-no-one-who-got-modernas-vaccine-trial-developed-severe-covid-19\"><span style=\"font-weight: 400;\">data is compelling<\/span><\/a><span style=\"font-weight: 400;\">. \u201cThe protection is far better than anyone anticipated,\u201d Paul Sax, an infectious disease specialist at Harvard, wrote recently in the <\/span><a href=\"https:\/\/www.nejm.org\/covid-vaccine\/faq\"><i><span style=\"font-weight: 400;\">New England Journal of Medicine<\/span><\/i><\/a><span style=\"font-weight: 400;\">. \u201cWhich is why many of us specialists in infectious diseases, virology immunology, and public health become downright giddy when asked to discuss the vaccine\u2019s efficacy.\u201d (\u201cGiddy\u201d is not a word you see often in the <\/span><i><span style=\"font-weight: 400;\">NEJM<\/span><\/i><span style=\"font-weight: 400;\">.) As for questions we can\u2019t answer yet, such as about the vaccine\u2019s safety years down the road, Sax points out that long-term side effects with vaccines are rare in general.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">But, in most cases, Chapman notes, facts alone have limited persuasive power.<\/span> <span style=\"font-weight: 400;\">\u201cIt\u2019s not always the case that presenting people with factual evidence is going to turn them around. Changing beliefs and changing behavior \u2014 there\u2019s no easy recipe for doing that. We\u2019re all very good at motivated reasoning and coming to believe the things that are consistent with what we wanted to believe.\u201d\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The best way for these conversations to begin is with listening, Chapman says, whether a state health department is building an educational app for historically vaccine-hesitant communities, or you&#8217;re trying to set the record straight about vaccine side effects during an informal chat with a friend. What are the person\u2019s (or group\u2019s) vaccine concerns? Can they be addressed directly?<\/span> <span style=\"font-weight: 400;\">The answers to these questions should inform how vaccine information is communicated.\u00a0\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">For example, one commonly raised concern about the COVID vaccines is how quickly they were developed and approved. The term \u201cOperation Warp Speed\u201d probably hasn\u2019t helped. Coined by the previous administration to describe an accelerated production strategy, the term only fueled worries about rushed development and insufficient clinical testing, likely helping to undermine public trust. <\/span><\/p>\n<p>\u201cObviously there\u2019s no getting around the fact that it\u2019s a new vaccine,\u201d says Chapman. \u201cBut the reason that we could develop vaccines in 10 months was in part because there\u2019s a decade of research on mRNA vaccines as a category, and if those vaccine developments hadn\u2019t been ongoing for years, this warp speed thing wouldn\u2019t have worked.\u201d<\/p>\n<p><span style=\"font-weight: 400;\">Consistency is also important in messaging. Confusing guidance on masks at the start of the pandemic, followed by the absence of a unified message from government leaders, opened the door for people to ignore the strong scientific evidence that wearing masks significantly helps reduce the spread of the virus. The COVID vaccine rollout offers an opportunity to course-correct. \u201cOne thing we can learn is: It\u2019s important that people at all levels of government are giving the same, evidence-based message,\u201d Chapman says.<\/span><\/p>\n<p>Of course, evidenced-based guidance may change if new evidence comes to light. That&#8217;s how scientific knowledge works, and you don&#8217;t need to pretend otherwise in order to advocate for listening to the science.<\/p>\n<p><b><i>Invoke the power of peer pressure<\/i><\/b><\/p>\n<p><span style=\"font-weight: 400;\">\u201cEverybody\u2019s doing it, you should too\u201d might not be a sophisticated line of reasoning, but psychologists and middle-schoolers alike can vouch for its effectiveness. If an airtight argument doesn\u2019t compel someone to get vaccinated, there\u2019s still a chance that seeing people get the vaccine, as well as hearing about their experiences, might do the trick.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cConforming really does feel good,\u201d says Catherine Sanderson, professor of psychology at Amherst College and author of <\/span><a href=\"https:\/\/www.amazon.com\/Why-We-Act-Turning-Bystanders\/dp\/0674241835#:~:text=Why%20We%20Act%20is%20a,including%20research%20by%20the%20author.&amp;text=In%20related%20chapters%2C%20Why%20We,done%20to%20improve%20the%20situation.\"><i><span style=\"font-weight: 400;\">Why We Act: Turning Bystanders into Moral Rebels<\/span><\/i><\/a><span style=\"font-weight: 400;\">.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">A <\/span><a href=\"https:\/\/www.jstor.org\/stable\/10.1086\/586910?seq=1#metadata_info_tab_contents\"><span style=\"font-weight: 400;\">classic study<\/span><\/a><span style=\"font-weight: 400;\"> on reusing hotel towels illustrates how health messaging can appeal to our desire to conform. In the study, guests were shown two versions of a sign asking them to reuse their towels to reduce water consumption. One message was just about saving the environment: \u201cYou can show your respect for nature and help save the environment by reusing your towels during your stay.\u201d The other one was less pure: \u201cJoin your fellow guests in helping to save the environment. Almost 75 percent of guests who are asked to participate in our new resource savings program do help by using their towels more than once.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The second message prevailed. \u201cThey found that people don\u2019t care about Mother Earth,\u201d says Sanderson. \u201cWhat they care about is that <\/span><i><span style=\"font-weight: 400;\">other<\/span><\/i><span style=\"font-weight: 400;\"> people care about Mother Earth. See the distinction?\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The <\/span><span style=\"font-weight: 400;\">allure of conformity is strongest for groups we identify with and care about, Sanderson says. So scrolling through vaccine selfies is more likely to sway someone if the arms in the photos belong to the right people.<\/span><\/p>\n<p><em><b>Target local messengers\u00a0<\/b><\/em><\/p>\n<p><span style=\"font-weight: 400;\">One way to motivate vaccination among particularly hesitant groups is to leverage the influence of trusted community leaders. In rural areas with low vaccination rates, Sanderson suggests local ministers. \u201cYou need to have role models who speak to a particular community [to say], \u2018I\u2019m a minister, I\u2019m a Republican, I\u2019m pro-life and I just got the vaccine.\u2019\u2019<\/span><\/p>\n<p><em><b>Don\u2019t ignore history<\/b><\/em><\/p>\n<p><span style=\"font-weight: 400;\"><a href=\"https:\/\/www.cdc.gov\/pcd\/issues\/2020\/20_0255.htm\">Black people are more likely<\/a> than other racial and ethnic groups <a href=\"https:\/\/www.cdc.gov\/pcd\/issues\/2020\/20_0255.htm\">to contract<\/a>, be hospitalized with and die of COVID. Yet, last month, about <a href=\"https:\/\/www.kff.org\/coronavirus-covid-19\/report\/kff-covid-19-vaccine-monitor-december-2020\/\">35 percent of Black Americans said<\/a> they didn\u2019t plan to get the vaccine, according to the KFF.\u00a0<\/span><\/p>\n<p>Context always matters, and it&#8217;s especially important here.<\/p>\n<p>For the Black community, vaccine hesitancy has deep roots, informed by first-hand experiences with a racially biased healthcare system as well as a history of nonconsensual participation in research, such as the 40-year-long Tuskegee experiment. Beginning in 1932, the United States Public Health Service deceptively enrolled hundreds of Black men in a study designed to &#8220;observe the natural history of untreated syphilis&#8221; under the guise of offering them free treatment for &#8220;bad blood.&#8221; The participants, who didn&#8217;t know they were in the study, weren&#8217;t offered treatment for syphilis. At the same time, they were discouraged from seeking outside medical care. Most participants had died, many from syphilis-related causes, by the time news of the experiment went public in 1972. The US government issued a formal apology in 1997.<\/p>\n<p>The Tuskegee experiment is <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4354806\/\">an egregious example<\/a> of Black people being exploited in the name of science research, but it&#8217;s not the only time it&#8217;s happened.<\/p>\n<p><span style=\"font-weight: 400;\">\u201cThose fears are well-founded,\u201d says Sanderson, and this is another area where messages from trusted spokespeople can be powerful. \u201cKamala Harris is a particularly important role model in this way,\u201d she says, referring to the vice-president\u2019s televised vaccination. \u201cI think if we can see other examples of people who are leaders in particular communities including communities of color, that would be profound.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Dr. <\/span><span style=\"font-weight: 400;\">Robert Drummond<\/span><span style=\"font-weight: 400;\">, a Black urgent care physician based in Los Angeles, has been leading regular <\/span><a href=\"https:\/\/www.instagram.com\/p\/CKSNrrNsEdV\/\"><span style=\"font-weight: 400;\">Instagram<\/span><\/a><span style=\"font-weight: 400;\"> Live sessions to educate the public about the vaccine and related topics, <a href=\"https:\/\/www.npr.org\/sections\/health-shots\/2021\/01\/19\/956015308\/you-cant-treat-if-you-cant-empathize-black-doctors-tackle-vaccine-hesitancy\">with an emphasis on reaching the Black community<\/a>. He answers questions \u2014 <\/span><i><span style=\"font-weight: 400;\">Is it true that the vaccine can cause sterilization in females? (No.)<\/span><\/i><span style=\"font-weight: 400;\"> \u2014 and responds to news of the day. Among the topics he covers are the vaccine\u2019s various side effects and why they happen. One follower posted that she was not planning to get the vaccine because of \u201ctrust issues,\u201d but now, since watching his videos, she and her family plan to get it.<\/span><\/p>\n<hr class=\"squiggle\" \/>\n<p><span style=\"font-weight: 400;\">As more of us get vaccinated and regain freedoms the unvaccinated can\u2019t enjoy, hesitancy might decrease. <\/span>\u201cIt may be that time is on our side,\u201d Chapman points out. \u201cOver time, each of us is going to know more and more people who have been vaccinated as the rollout continues. Vaccine hesitancy may drop a bit more as people see it more and more normalized and more people getting it.\u201d<\/p>\n<hr \/>\n<h1 class=\"p1\" style=\"text-align: center;\"><span class=\"s1\">Ready to book a doctor&#8217;s appointment?
