{"id":18984,"date":"2021-01-27T15:44:56","date_gmt":"2021-01-27T20:44:56","guid":{"rendered":"http:\/\/thepapergown.zocdoc.com\/?p=18984"},"modified":"2023-03-06T15:21:48","modified_gmt":"2023-03-06T20:21:48","slug":"when-it-comes-to-vaccine-eligibility-what-does-high-risk-mean","status":"publish","type":"post","link":"https:\/\/www.zocdoc.com\/blog\/guides\/when-it-comes-to-vaccine-eligibility-what-does-high-risk-mean\/","title":{"rendered":"When It Comes to Vaccine Eligibility, What Does &#8220;High-Risk&#8221; Mean?"},"content":{"rendered":"<p><span style=\"font-weight: 400;\">With the vaccine rollout chugging along, many states are starting to vaccinate residents considered \u201chigh-risk\u201d due to underlying illness. If you have a heart issue or diabetes, you may already know that you\u2019re part of this group and, as a result, allowed to jump the vaccine line. <\/span><span style=\"font-weight: 400;\">But what if someone has an autoimmune disease or a developmental disorder? Which other conditions, besides the most frequently mentioned ones, affect vaccination eligibility?<\/span><\/p>\n<p><span style=\"font-weight: 400;\">While it\u2019s important to know if you have a heightened risk of becoming severely ill from COVID, figuring out if your health status qualifies you for early vaccination for exactly that reason isn\u2019t as simple as you might think. But understanding why that&#8217;s the case, and knowing where to look for guidance, can make the situation easier to navigate.<\/span><\/p>\n<hr \/>\n<h3>It\u2019s up to the states<\/h3>\n<p><span style=\"font-weight: 400;\">The federal government has issued guidelines on the order of priority for vaccination, its health officials periodically chime in with updated recommendations. But they\u2019re only recommendations, not federal mandates.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Every state has had to come up with its own phased distribution plan to determine how and when to administer the vaccine. People who have underlying medical conditions that put them at an increased risk of severe complications from COVID-19 are considered \u201chigh-risk.\u201d These underlying conditions are also called comorbidities. For someone to have severe COVID complications (or illness), per the <\/span><a href=\"https:\/\/www1.nyc.gov\/site\/doh\/covid\/covid-19-prevention-and-care.page\"><span style=\"font-weight: 400;\">New York City Department of Health<\/span><\/a><span style=\"font-weight: 400;\">, it means they might need \u201chospitalization, intensive care or a ventilator to help them breathe, or that they may even die.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cHigh-risk\u201d individuals are uniformly treated as a group that gets priority over the general public, but the precise definition of the term varies. While most states are using the <a href=\"https:\/\/www.cdc.gov\/coronavirus\/2019-ncov\/need-extra-precautions\/people-with-medical-conditions.html\">CDC\u2019s list<\/a><\/span><span style=\"font-weight: 400;\"> of conditions that carry an increased risk of severe illness from COVID <\/span><span style=\"font-weight: 400;\">in their vaccine plan, other states have yet to define which chronic conditions make people eligible for early vaccination.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cMuch of the confusion stems from each state rolling out their plan a little bit differently,\u201d says Dr. Amy Mullins, medical director of quality and science at theAmerican Academy of Family Physicians. \u201cAnd, within states, each county may be implementing their plan differently.\u201d<\/span><\/p>\n<blockquote><p><span style=\"font-weight: 400;\">Many others are <\/span><a href=\"https:\/\/www.tn.gov\/content\/dam\/tn\/health\/documents\/cedep\/novel-coronavirus\/COVID-19_Vaccine_Provider_FAQs.pdf\"><span style=\"font-weight: 400;\">taking a similar approach<\/span><\/a><span style=\"font-weight: 400;\"> and relying on the honor system.<\/span><\/p><\/blockquote>\n<p><span style=\"font-weight: 400;\">For example, New York State won\u2019t start vaccinating people with underlying health conditions <\/span><a href=\"https:\/\/www1.nyc.gov\/site\/doh\/covid\/covid-19-vaccine-eligibility.page\"><span style=\"font-weight: 400;\">until the spring<\/span><\/a><span style=\"font-weight: 400;\">, and has yet to determine which ones make its list. Texas is already vaccinating people with at least one high-risk medical condition. <\/span><a href=\"https:\/\/www.dshs.state.tx.us\/coronavirus\/immunize\/vaccine.aspx\"><span style=\"font-weight: 400;\">The Texas Department of State Health Services<\/span><\/a><span style=\"font-weight: 400;\"> lists qualifications for phase 1B \u2014 including cancer, chronic kidney disease and sickle cell disease \u2014 yet it also includes language suggesting other chronic conditions might qualify as well.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cThe qualifier in the language does imply there are other chronic conditions that certainly put people at risk for developing severe complications from COVID-19,\u201d says<\/span> <span style=\"font-weight: 400;\">Dr. Sherri Onyiego, <\/span><span style=\"font-weight: 400;\">division director for nutrition and chronic disease prevention at Harris County Public Health in Texas.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Texas\u2019 list of eligible high-risk conditions is based on the most common health issues doctors have seen with COVID-19, and ones that current data support, she adds. As the science evolves, so too will states&#8217; decisions regarding vaccine priority. Just this past week, Texas added Down&#8217;s syndrome to its 1b eligibility criteria. This change came weeks after the CDC updated <\/span><a href=\"https:\/\/www.cdc.gov\/coronavirus\/2019-ncov\/need-extra-precautions\/people-with-medical-conditions.html\"><span style=\"font-weight: 400;\">its own list<\/span><\/a><span style=\"font-weight: 400;\"> of conditions that carry an increased risk of severe illness.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">A <\/span><a href=\"https:\/\/www.acpjournals.org\/doi\/10.7326\/M20-4986\"><span style=\"font-weight: 400;\">recent study<\/span><\/a><span style=\"font-weight: 400;\"> found that people with Down\u2019s syndrome are 10 times more likely to die from COVID-19 because of <\/span><span style=\"font-weight: 400;\">abnormal immune responses, congenital heart disease and lung abnormalities commonly associated with the disorder.<\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><\/p>\n<h3>COVID and autoimmune diseases<\/h3>\n<p><span style=\"font-weight: 400;\">People who are immunocompromised are at high risk of developing complications from COVID-19 as well, according to the CDC. Though, on their vaccine priority lists, many states don\u2019t include conditions that can be linked to a weaker immune system like lupus.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Dr. Andres Splenser, an endocrinologist in Houston in private practice, says it\u2019s important to distinguish between<\/span> <span style=\"font-weight: 400;\">having an autoimmune disorder and being immunocompromised. \u201cWith an autoimmune disorder, your immune system works too hard,\u201d he says. \u201cIt doesn\u2019t mean you\u2019re immunocompromised.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">However, some people with autoimmune diseases do take drugs that suppress their immune function, which could make them immunocompromised. <\/span><span style=\"font-weight: 400;\">What\u2019s more, <\/span><a href=\"http:\/\/www.medrxiv.org\/content\/10.1101\/2020.12.10.20247205v4\"><span style=\"font-weight: 400;\">evolving research suggests<\/span><\/a><span style=\"font-weight: 400;\"> that autoantibodies, which play a role in autoimmune diseases, may also play a part in some of the worst cases of COVID-19. With autoimmune diseases, the immune system misfires, producing antibodies against itself and causing damage to organs throughout the body.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cThere is tentative research that people with autoimmune conditions could get sicker from COVID,\u201d says Dr. Natasha Bhuyan, a family physician at One Medical in Phoenix.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Robert Atmar, professor of Medicine at Baylor College of Medicine, says that, ultimately, it will depend on how individual states identify persons at risk and include them for prioritized vaccination.<\/span><\/p>\n<h3>Where to look to see if you\u2019re eligible for early vaccination<\/h3>\n<p><span style=\"font-weight: 400;\">If you have a chronic health condition and you want to know if you\u2019re eligible to receive the vaccine early in your state, you can:<\/span><\/p>\n<ol>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Find your area\u2019s vaccine plan.<\/b><span style=\"font-weight: 400;\"> Look at your county, city or state\u2019s health department website to find information on its vaccine rollout plan and its list of qualifying health conditions.<\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Check for any CDC updates.<\/b><span style=\"font-weight: 400;\"> Consult the CDC\u2019s website for its most up-to-date list of conditions that carry a heightened risk of severe illness from COVID-19. If the CDC has added a high-risk condition that\u2019s not on your state\u2019s vaccine priority list, you can lobby local officials to update their list.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Talk to your doctor.<\/b><span style=\"font-weight: 400;\"> During routine appointments, Splenser has been making sure his high-risk patients know they\u2019re eligible, and if he\u2019s not sure, he encourages patients with chronic conditions to contact their primary care doctor for guidance. \u201cThere are some gray areas,\u201d he says, \u201cso if you\u2019re not sure, definitely ask your healthcare provider.\u201d<\/span><\/li>\n<\/ol>\n<p><span style=\"font-weight: 400;\">Onyiego<\/span><span style=\"font-weight: 400;\"> says Harris County (in Texas) doesn\u2019t currently require documentation to prove an eligible health condition. Many others are <\/span><a href=\"https:\/\/www.tn.gov\/content\/dam\/tn\/health\/documents\/cedep\/novel-coronavirus\/COVID-19_Vaccine_Provider_FAQs.pdf\"><span style=\"font-weight: 400;\">taking a similar approach<\/span><\/a><span style=\"font-weight: 400;\"> and relying on the honor system. Splenser adds that while many people are eager to get the vaccine, it\u2019s important to let the most vulnerable people go first.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cWhen there\u2019s low supply, we have to target healthcare workers, front line workers and people who are really at risk of getting a severe infection first. We really have to step back and say, who is it that\u2019s dying of COVID, and those are the people who get it first.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In the meantime, Mullins says, it\u2019s important to continue practicing the safety measures we\u2019ve followed for months. \u201cAs we all await our turn to be vaccinated, it\u2019s more important than ever to wash hands, maintain physical distancing and wear a mask,\u201d she says.<\/span><\/p>\n<hr \/>\n<h1 class=\"p1\" style=\"text-align: center;\"><span class=\"s1\">Ready to book a doctor&#8217;s appointment?
