{"id":19228,"date":"2021-09-13T12:52:28","date_gmt":"2021-09-13T17:52:28","guid":{"rendered":"https:\/\/thepapergown.zocdoc.com\/?p=19228"},"modified":"2023-03-06T11:55:45","modified_gmt":"2023-03-06T16:55:45","slug":"how-can-therapists-reach-a-wider-array-of-black-patients","status":"publish","type":"post","link":"https:\/\/www.zocdoc.com\/blog\/healthcare-trends\/how-can-therapists-reach-a-wider-array-of-black-patients\/","title":{"rendered":"How Can Therapists Reach a Wider Array of Black Patients?"},"content":{"rendered":"<p><span style=\"font-weight: 400;\">When patients and providers don\u2019t communicate well, health issues are more likely to be misdiagnosed, improperly treated or overlooked entirely. And patients are more likely to skip out on care. One area with notable communication gaps, research suggests, is mental healthcare in the Black community.\u00a0<\/span><\/p>\n<p><a href=\"https:\/\/ssw.uga.edu\/people\/name\/rosalyn-denise-campbell\/\"><span style=\"font-weight: 400;\">Rosalyn Denise Campbell<\/span><\/a><span style=\"font-weight: 400;\">, <\/span><span style=\"font-weight: 400;\">an associate professor at the University of Georgia\u2019s School of Social Work, recently co-authored a study exploring how Black Americans describe mental health issues. <\/span><a href=\"https:\/\/academic.oup.com\/hsw\/article-abstract\/45\/1\/55\/5716283?redirectedFrom=fulltext\"><span style=\"font-weight: 400;\">The findings<\/span><\/a><span style=\"font-weight: 400;\"> revealed a generational divide: While younger Black patients tend to talk about mental health in the same clinical terms therapists and other mental health providers use, older Black patients default to more colloquial language.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cMany Black folks talk about their psychological distress in physical terms,\u201d says <\/span><a href=\"https:\/\/ssw.uga.edu\/people\/name\/rosalyn-denise-campbell\/\"><span style=\"font-weight: 400;\">Campbell<\/span><\/a><span style=\"font-weight: 400;\">. \u201cIf you ask them how they\u2019re feeling and they say \u2018tired,\u2019 they may not mean that they are not getting enough rest; they may mean that they are \u2018stressed out\u2019 or \u2018depressed.\u2019\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The study emphasizes the need for providers to be familiar with small but important cultural nuances in the way different patient populations label symptoms or interpret diagnoses, and then adapt their communication habits accordingly. Otherwise, critical health information may be lost in translation.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Campbell\u2019s firsthand healthcare experiences shaped her research interests. She dealt with anxiety and depression. But she didn\u2019t realize what was wrong until she started therapy as an adult, because she didn\u2019t have the right labels for her emotions. Today, Campbell studies how Black people<\/span> interact with <span style=\"font-weight: 400;\">the mental healthcare system, with the goal of \u201cinforming how providers design and deliver services that holistically meet the needs of this group.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">For her study, Campbell and colleagues surveyed nearly 400 members of a large Black church in the Midwest. \u201cThis church actually sought me out,\u201d says Campbell. After several mental health clinics in the area closed, the church wanted to develop effective resources for people in the community. \u201cThey were thinking of expanding their existing counseling ministry to a counseling center, but wanted more information before they moved forward. One of their mental health counselors knew I did research in this area.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In particular, the church\u2019s counseling team wanted to make sure they were able to adequately discuss depression, trauma, anxiety, grief and loss with the church\u2019s congregants.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cThe counseling team saw the church as a welcome alternative given that they were located in the community and a trusted institution that felt not only comfortable and safe enough to use, but also that the clinicians there would understand their experiences as African Americans, Christians and\/or Black Christians,\u201d says Campbell.\u00a0<\/span><\/p>\n<h3>Surveying the community<\/h3>\n<p><span style=\"font-weight: 400;\">Campbell&#8217;s research showed that middle-aged and younger Black adults were comfortable using clinical terms when discussing mental health; they embraced diagnostic labels like depression and anxiety. <\/span><span style=\"font-weight: 400;\">But older Black adults tended to rely on euphemisms instead. They called depression \u201cthe blues,\u201d for example, and described anxiety as \u201cnerves.\u201d\u00a0<\/span><span style=\"font-weight: 400;\">And only clinicians called \u201ctrauma\u201d by its name. \u201c [In general, community members] were more likely to describe the nature or circumstances of what they experienced and its impact,\u201d Campbell says.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In her own work, <\/span><a href=\"https:\/\/www.justdavia.com\/about\"><span style=\"font-weight: 400;\">Davia Roberts<\/span><\/a><span style=\"font-weight: 400;\">, a therapist and consultant in Washington, DC, has noticed similar language trends in Black patients. <\/span><span style=\"font-weight: 400;\">\u201cWhen you have communities that have their own distinct cultures, naturally we speak in different ways,\u201d she says. \u201cAs practitioners, we have files and paperwork that list all these \u2018proper\u2019 terms, but they don\u2019t account for cultural nuances.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Plus, in communities of color, seeing a therapist still carries a stigma \u2014 despite conversations about mental health becoming more prevalent. As a result, many Black people still suffer in silence.\u00a0<\/span>Not to mention, many Black people remain mistrustful of the medical establishment on account of <a href=\"https:\/\/www.theguardian.com\/society\/2021\/aug\/20\/black-americans-covid-19-vaccine-hesitancy-mistrust\">historical exploitation and nonconsensual experimentation<\/a>. If a Black client shares concerns about suicidal ideation, says Roberts, they may have a legitimate fear of being hospitalized or having their children taken away from them as a result of disclosing those thoughts.<\/p>\n<p><span style=\"font-weight: 400;\">There are ways to bridge this language gap, experts say. For starters, Campbell is teaching her social work students about cultural competence, a concept gaining steam in the patient-care universe. Researchers define <\/span><a href=\"https:\/\/www.commonwealthfund.org\/sites\/default\/files\/documents\/___media_files_publications_fund_report_2002_oct_cultural_competence_in_health_care__emerging_frameworks_and_practical_approaches_betancourt_culturalcompetence_576_pdf.pdf\"><span style=\"font-weight: 400;\">cultural competence<\/span><\/a><span style=\"font-weight: 400;\"> in healthcare as the \u201cability of providers and organizations to effectively deliver healthcare services that meet the social, cultural and linguistic needs of patients.\u201d\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cThe way our healthcare system is set up, it\u2019s very much about fitting people into boxes. And when they don\u2019t fit into boxes, then we say, \u2018We can\u2019t help you,\u201d Campbell says. \u201cWhat it comes down to is doing really good assessments and asking really good questions.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Campbell teaches her students to take a slower, more intentional approach when it comes to conversations with Black patients. <\/span><span style=\"font-weight: 400;\">Instead of asking patients to fill out checklists describing their mental health needs, Roberts puts this approach into practice by asking patients clarifying questions and offering them context.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cI\u2019ll ask, \u2018Are you experiencing anxiety? Are you experiencing suicidal thoughts?\u2019\u201d she shares. And when the clients say yes or no, I clarify and say \u2018sometimes this can look like feeling nervous, or having thoughts like, <\/span><i><span style=\"font-weight: 400;\">I think it would be easier if I just didn\u2019t wake up in the morning<\/span><\/i><span style=\"font-weight: 400;\">.\u2019 Typically, that\u2019s when our conversation changes.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In the grand scheme of things, Campbell hopes her study can help clinicians understand that a wide array of demographic and individual factors affect mental well-being and how people talk about it \u2014 and that Black people aren\u2019t a monolith.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">As for patients, seeking help from resources such as <\/span><a href=\"https:\/\/therapyforblackgirls.com\/\"><span style=\"font-weight: 400;\">Therapy for Black Girls<\/span><\/a><span style=\"font-weight: 400;\">, <\/span><a href=\"https:\/\/therapyforblackmen.org\/\"><span style=\"font-weight: 400;\">Therapy for Black Men<\/span><\/a><span style=\"font-weight: 400;\">, <\/span><a href=\"https:\/\/www.inclusivetherapists.com\/\"><span style=\"font-weight: 400;\">Inclusive Therapists<\/span><\/a><span style=\"font-weight: 400;\"> and <\/span><a href=\"https:\/\/therapyforqpoc.com\/\"><span style=\"font-weight: 400;\">Therapy for Queer People of Color<\/span><\/a><span style=\"font-weight: 400;\"> is a great way to find affirming practitioners who use culturally competent treatment. That being said, Black therapists, who currently make up only <\/span><a href=\"https:\/\/www.apa.org\/workforce\/data-tools\/demographics\"><span style=\"font-weight: 400;\">3% of the US psychology workforce<\/span><\/a><span style=\"font-weight: 400;\">, are busier than ever. <\/span><\/p>\n<p>Campbell\u2019s message to patients? \u201cRemember you\u2019re the expert on yourself.
