{"id":19431,"date":"2021-11-19T11:55:36","date_gmt":"2021-11-19T16:55:36","guid":{"rendered":"https:\/\/thepapergown.zocdoc.com\/?p=19431"},"modified":"2023-03-06T11:45:20","modified_gmt":"2023-03-06T16:45:20","slug":"flossing-debate","status":"publish","type":"post","link":"https:\/\/www.zocdoc.com\/blog\/advice\/flossing-debate\/","title":{"rendered":"What&#8217;s With The Debate Over Flossing?"},"content":{"rendered":"<p><span style=\"font-weight: 400;\">Ever since <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/10686903\/\"><span style=\"font-weight: 400;\">1819,<\/span><\/a><span style=\"font-weight: 400;\"> when a New Orleans dentist named Levi Spear Parmly began recommending his patients run a waxen silk thread between their teeth, flossing has been a topic of interest for oral health experts. However, given the lack of large studies surrounding the dental cleaning technique, there&#8217;s been great debate over it over the years.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cI think this whole debate or controversy stems from the fact that there really haven&#8217;t been good studies that prove the use of floss,\u201d says dentist, Dr. Frank Scannapieco, who&#8217;s also chair and professor of oral biology in the University at Buffalo School of Dental Medicine. \u201cIt&#8217;s not like there&#8217;s proof it doesn&#8217;t work, there just haven\u2019t been rigorous studies showing that it works.\u201d\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Regardless, less than half of all Americans floss daily, according to a <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC6434526\/\"><span style=\"font-weight: 400;\">CDC survey. <\/span><\/a><span style=\"font-weight: 400;\">Dental experts still want to see more people adopt the habit. Here\u2019s what to know about the debate over flossing.\u00a0<\/span><\/p>\n<hr class=\"squiggle\" \/>\n<h3>The evidence<\/h3>\n<p><span style=\"font-weight: 400;\">The number of studies on flossing are limited.<\/span><span style=\"font-weight: 400;\">\u00a0In 2011, the Cochrane Collaboration, a group of researchers that analyzes studies to answer health questions, conducted a <\/span><a href=\"https:\/\/www.cochranelibrary.com\/cdsr\/doi\/10.1002\/14651858.CD008829.pub2\/full\"><span style=\"font-weight: 400;\">review<\/span><\/a><span style=\"font-weight: 400;\"> of the evidence on flossing. They looked at 12 studies and found that there was only weak evidence that flossing could reduce plaque. In 2019, a <\/span><a href=\"https:\/\/doi.org\/10.1002%2F14651858.CD012018.pub2\"><span style=\"font-weight: 400;\">follow-up review<\/span><\/a><span style=\"font-weight: 400;\"> found that the evidence in favor of flossing was minimal.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In 2015, flossing took another hit when the <\/span><a href=\"https:\/\/health.gov\/our-work\/nutrition-physical-activity\/dietary-guidelines\/previous-dietary-guidelines\/2015\"><span style=\"font-weight: 400;\">United States Dietary Guidelines for Americans,<\/span><\/a><span style=\"font-weight: 400;\"> published every five years, didn\u2019t include flossing in its list of recommendations as it had in years prior. The following year, the Associated Press examined 25 studies on flossing and <\/span><a href=\"https:\/\/apnews.com\/article\/f7e66079d9ba4b4985d7af350619a9e3\"><span style=\"font-weight: 400;\">published<\/span><\/a><span style=\"font-weight: 400;\"> an article claiming there was no scientific evidence for flossing.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u00a0<\/span><span style=\"font-weight: 400;\">\u201cThe research tells us that contrary to traditional belief, flossing is the worst interdental cleaning method available,\u201d says Georgios Kotsakis, an associate professor in the periodontics department of the University of Texas at San Antonio. He adds that only toothpicks are worse than floss when it comes to cleaning between teeth.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">But some experts point out that these studies have their flaws. For one, they ask participants to self-report how much they floss, which could yield unreliable data: One <\/span><a href=\"https:\/\/www.reuters.com\/article\/us-usa-healthcare-flossing\/one-fourth-of-americans-lie-to-dentists-about-flossing-survey-idUSKBN0P32BJ20150623\"><span style=\"font-weight: 400;\">poll<\/span><\/a><span style=\"font-weight: 400;\"> found that one out of four people lie about their flossing habits. Plus, people\u2019s flossing <\/span><a href=\"https:\/\/www1.nyc.gov\/assets\/doh\/downloads\/pdf\/hca\/sdc-brush-floss.pdf\"><span style=\"font-weight: 400;\">techniques<\/span><\/a><span style=\"font-weight: 400;\"> and preferences vary significantly. Two people might floss, but one person\u2019s flossing might be effective while another person\u2019s technique is not.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Will there ever be a gold standard study to definitively show that flossing works? It\u2019s unlikely, Scannapieco says.\u00a0<\/span><span style=\"font-weight: 400;\">Floss, like toothbrushes and a whole host of other products we use every day, might be too embedded in our culture to get studied at length. Flossing originated over 200 years ago, before scientists began using randomized clinical trials to assess the effectiveness of medical treatments.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cThere also haven\u2019t really been good studies to prove the value of the toothbrush, but that doesn&#8217;t mean that doesn&#8217;t work,\u201d Scannapieco says.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Most clinical trials today also tend to receive funding from companies with a vested interest in learning whether a product works, and many people already assume flossing and toothbrushing are effective.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cA lot of the impetus for studying a product is whether or not a company can make money on it,\u201d Scannapieco says. \u201cI don&#8217;t think companies are out to prove whether or not floss works because they won&#8217;t recoup the cost of running a big trial.\u201d\u00a0<\/span><\/p>\n<h3>The takeaway<\/h3>\n<p><span style=\"font-weight: 400;\">So should we listen to the research telling us to throw out our floss? Not so fast, dentists say.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cFlossing improves oral hygiene when added to toothbrushing alone,\u201d Kotsakis says. But for a bigger boost to oral health, he suggests incorporating tools beyond flossing. \u201cAlternatives such as waterjets and interdental brushes help reduce inflammation much better.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">For patients who struggle with flossing\u2014whether they find it difficult and time consuming, or their teeth are too crowded to easily floss between them\u2014Kotsakis also recommends those alternatives, along with other oral irrigators and dental tape. <\/span><span style=\"font-weight: 400;\">S<\/span><span style=\"font-weight: 400;\">cannapieco says it makes practical sense to clean between the teeth, and he always recommends flossing to his patients.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cMost dentists or hygienists will tell you flossing helps,\u201d Scannapieco says. \u201cYou have to clean in between your teeth somehow.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Scannapieco has noticed that his patients who take the time to floss correctly tend to have better gum health, less bad breath and decreased inflammation. Flossing to remove bacteria at the gum line could reduce the risk of gum inflammation and ultimately prevent plaque from forming.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cIf you were to do a well-designed, large scale, placebo-controlled clinical trial, it would show that floss has benefits,\u201d Scannapieco says. \u201cMaybe someday somebody will do that study to correct this misunderstanding.\u201d<\/span><\/p>\n<hr \/>\n<h1 class=\"p1\" style=\"text-align: center;\"><span class=\"s1\">Ready to book a dentist&#8217;s appointment?
