If you run into an emergency, having the right files on hand can be life-saving.<\/p>\n<p>Unfortunately, right now, this is all much easier said than done.<\/p>\n<p><span style=\"font-weight: 400;\">Many primary care providers use patient portals, where they store records in a single spot for easy access. But if you see a specialist or get blood tests done at an independent lab, your records might not be immediately shared with your PCP \u2014 without your input, at least. Furthermore, life changes, like moving or changing doctors, can also put the state of your records in flux.\u00a0<\/span><\/p>\n<p>W<span style=\"font-weight: 400;\">hile there&#8217;s been a <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3088297\/\">push<\/a> by the government and medical community to create an umbrella, centralized medical records system in the last few decades, efforts have failed. Such a system would pull in all info from doctor&#8217;s offices across the country for easy access by both patients and providers. But currently, d<\/span><span style=\"font-weight: 400;\">ata is spread out across a labyrinth of providers, medical systems and private labs. Often, it is entrenched in clunky, outdated systems.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u00a0\u201cThere are technical and business challenges with a centralized system,&#8221; says Dr. Steven Waldren, Chief Medical Informatics Officer at the American Academy of Family Physicians. &#8220;I do think it is inevitable for a good chunk of patients, but not anytime soon.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u00a0<\/span><span style=\"font-weight: 400;\">In the absence of a centralized system, for now it\u2019s up to patients to create their own personal record keeping system. Here\u2019s how to create one that works for you.<\/span><\/p>\n<hr class=\"squiggle\" \/>\n<h3>Decide what data you need.<\/h3>\n<p><span style=\"font-weight: 400;\">Make a list of all the information you need to gather. Experts suggest patients gather personal medical records dating back as far as you can find. Below<\/span><span style=\"font-weight: 400;\">\u00a0are some examples of the types of documents you may want to include.\u00a0<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">History of any health emergencies (i.e. broken bones).\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Childbirth history.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Vaccination history.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Health screenings (i.e. blood pressure, cholesterol and blood sugar readings).<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Cancer screenings (i.e. pap smears, mammograms, colonoscopies).\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Blood and urine test results.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Results from MRIs or X-rays.\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Surgery history.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Hearing and vision test results.\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Info on any current\/former medications or supplements.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Medical bills and insurance information.\u00a0<\/span><\/li>\n<\/ul>\n<h3>Request your records.<span style=\"font-weight: 400;\">\u00a0<\/span><\/h3>\n<p>If your PCP uses an online portal, you can likely find a lot of the data you need there. If you can&#8217;t find everything you need, <a href=\"https:\/\/www.zocdoc.com\/blog\/guides\/a-guide-to-getting-test-results-from-your-doctor\/\">check in<\/a> with your doctor about records. (Check out our <a href=\"https:\/\/www.zocdoc.com\/blog\/guides\/a-guide-to-getting-test-results-from-your-doctor\/\"><em>Guide to Getting Test Results From Your Doctor.<\/em>)<\/a><\/p>\n<p><span style=\"font-weight: 400;\">Under the <a href=\"https:\/\/www.hhs.gov\/hipaa\/for-professionals\/privacy\/guidance\/access\/index.html\">Health Insurance Portability and Accountability Act (HIPAA)<\/a>, most medical providers are required to give you requested copies of medical records <\/span><span style=\"font-weight: 400;\">within 30 days.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">You might receive <\/span><a href=\"https:\/\/www.hhs.gov\/hipaa\/for-professionals\/faq\/570\/does-hipaa-permit-health-care-providers-to-use-email-to-discuss-health-issues-with-patients\/index.html\"><span style=\"font-weight: 400;\">records via email <\/span><\/a><span style=\"font-weight: 400;\">or be asked to pick up paper copies at your doctor&#8217;s office. Keep in mind that you can\u2019t get access to certain information, like <\/span><span style=\"font-weight: 400;\"><a href=\"https:\/\/www.zocdoc.com\/blog\/guides\/you-can-now-read-doctors-notes-heres-what-that-means\/\">notes<\/a> from psychotherapy sessions<\/span><span style=\"font-weight: 400;\">, in the US.\u00a0<\/span><\/p>\n<h3>Be mindful of the medium.<\/h3>\n<p><span style=\"font-weight: 400;\">\u201cThere\u2019s not really a right or wrong way for patients to track their health information,\u201d says Lesley Kadlec, Director of Policy and State Advocacy Engagement for the American Health Information Management Association. <\/span><span style=\"font-weight: 400;\">\u201cIt\u2019s a very personal thing. It\u2019s whatever format works for you.\u201d <\/span><\/p>\n<p><span style=\"font-weight: 400;\">All patients, however, should make sure caregivers and close family members know how and where information is stored in the case of any emergency.\u00a0<\/span><\/p>\n<p><b>Paper-based:<\/b> If you like having tangible proof of your records, you might want to invest in a paper-based organization system<span style=\"font-weight: 400;\">. One drawback of this format is the lack of a control f+ function: it can be hard to find what you\u2019re looking for when you need it. <\/span><\/p>\n<p>Waldren recommends sorting information <span style=\"font-weight: 400;\">sorting information chronologically, by date, with the most recent records first. Invest in a nice binder, make a title<\/span><span style=\"font-weight: 400;\"> page with your name, birthday, blood type, any diagnoses and emergency contacts. Include separate sections for bills and insurance documents. For security, keep the binder in a fireproof safe or box.\u00a0<\/span><\/p>\n<p><b>Going digital:<\/b><span style=\"font-weight: 400;\"> Searches can be easier within a digital system, which can make sharing info with your health care providers much easier.\u00a0 If you choose to go digital, experts recommend sorting information by date.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">You can easily save your records on your computer in a set of organized folders, or through services like Dropbox or Google Docs. But there are plenty of other user-friendly platforms out there made for storing medical records, like <\/span><a href=\"https:\/\/www.mychart.com\"><span style=\"font-weight: 400;\">MyChart<\/span><\/a><span style=\"font-weight: 400;\">.\u00a0<\/span><\/p>\n<h3>Create a new habit of asking for records.<\/h3>\n<p><span style=\"font-weight: 400;\">Once you\u2019ve created a system, commit to getting a copy of test results or appointment notes every time you get medical care. Then you can easily add to your binder or your electronic filing system as you go. \u201cKeeping information up to date is important,\u201d <\/span><span style=\"font-weight: 400;\">Kadlec<\/span><span style=\"font-weight: 400;\"> says.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">If you have a primary care doctor you\u2019re sticking with, who uses a patient portal, and you\u2019re relatively healthy and require little care, Waldren says another tactic is to simply use that portal as your storage system.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cI try to make sure all my data flows into my primary care physician,\u201d Waldren says, \u201cI can use that data to flow out to anyone else.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Whatever system you choose, talk to your primary care doctor first because they might have ideas on how best to organize and store your data, says Waldren.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cThe vast majority of primary care providers really feel like [they can be aggregators of data],\u201d he says. \u201cSo, take that into account as you organize your information.\u201d<\/span><\/p>\n<hr \/>\n<h1 class=\"p2\" style=\"text-align: center;\"><span class=\"s1\">Ready to book a doctor&#8217;s appointment?
