It Was Something Else."},"content":{"rendered":"<p><span style=\"font-weight: 400;\">I\u2019m familiar with sadness that makes your bones cold. For years, I thought it was just severe depression. Since I was a teen, it came in waves, up until this past winter, when I entered an especially dark place: In my Brooklyn apartment, rocked by miserable weather and a seemingly never-ending pandemic, I <\/span><i><span style=\"font-weight: 400;\">really<\/span><\/i><span style=\"font-weight: 400;\"> didn\u2019t want to live anymore.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">I\u2019d been prescribed a popular antidepressant in 2019 to help with anxiety and depression. But in 2021, after reaching the maximum dosage, it was clear that something had to give. I turned to my psychiatrist. We had noticed ups and downs in my mood in the past, but hadn\u2019t really dug into them.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">After a few months of mood tracking, I was diagnosed with bipolar II \u2014 a subtype of bipolar disorder I never knew existed. The massive mood swings are a key element of the disorder, and part of why \u201cregular\u201d depression is also called \u201cunipolar.\u201d While bipolar I is characterized by intense, euphoric manic episodes that can send people to the hospital, people with bipolar II experience more severe depression and short \u201chypomanic\u201d episodes.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">While both types of mania can make sufferers feel \u201clike the Energizer Bunny,\u201d says Dr. Daniel Kimmel, a <\/span><span style=\"font-weight: 400;\">Manhattan-based psychiatrist, neuroscientist and professor at Columbia University, the difference is in \u201chow interfering it is.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cPeople having a hypomanic episode \u2014 it will affect them, but they&#8217;re able to keep going on with their lives, with work, they&#8217;re able to see their friends, they&#8217;re able to do hobbies,\u201d he says.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">It\u2019s likely why I never suspected I had bipolar disorder. I\u2019ve always been a bit of a neurotic overachiever, my life marked by periods of elevated productivity, impulsivity and creativity, speckled with harrowing, dampening depression. I often made a lot of grand plans, only to not necessarily follow through once the depression came around. Everything made sense once I was able to name it.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Experts estimate that at least <\/span><a href=\"https:\/\/www.dbsalliance.org\/education\/bipolar-disorder\/bipolar-disorder-statistics\/\"><span style=\"font-weight: 400;\">5.7 million Americans<\/span><\/a><span style=\"font-weight: 400;\"> know they have bipolar disorder. I\u2019m telling my story because one, people get silent when I bring it up, and two, misconceptions about the illness can delay treatment \u2014 which can make an already deadly condition deadlier.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cBy no means is it a morbid sentence,\u201d Kimmel says.\u201cIt&#8217;s a controllable, livable disorder.\u201d<\/span><\/p>\n<hr class=\"squiggle\" \/>\n<h3>The \u2018B\u2019 Word<\/h3>\n<p><span style=\"font-weight: 400;\">When I realized I may be bipolar, first, I was ashamed. No one wants to ever say the \u201cB\u201d word.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">When it appears in movies or TV shows, <\/span><a href=\"https:\/\/www.vh1.com\/news\/8h1wsi\/bipolar-disorder-hollywood-misconceptions\"><span style=\"font-weight: 400;\">bipolar disorder<\/span><\/a><span style=\"font-weight: 400;\"> is often highly exaggerated, depicting people who are dangerous or dysfunctional.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">That\u2019s not to say the disorder can\u2019t be debilitating; it absolutely can be. But it\u2019s not always so black and white. Fortunately, medical providers have come a long way in understanding how it functions.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In 1960, the National Institute of Mental Health looked at data from its hospitalizations and split a group of patients into three categories: patients who experienced depression, patients who experienced major depression and mania, and patients who experienced depression and mild mania (or hypomania).<\/span><\/p>\n<p><span style=\"font-weight: 400;\">While the first two groups were recognized as unipolar and bipolar depression, the third was simply categorized as \u201catypical bipolar.\u201d It wasn\u2019t until 1980 that bipolar disorder finally made it into the third edition of the <\/span><i><span style=\"font-weight: 400;\">Diagnostic and Statistical Manual of Mental Disorders (DSM-III)<\/span><\/i><span style=\"font-weight: 400;\">. Bipolar II, \u201catypical bipolar,\u201d didn\u2019t make it in until 1994, when the <\/span><i><span style=\"font-weight: 400;\">DSM-IV<\/span><\/i><span style=\"font-weight: 400;\">\u00a0was published.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Due to some overlap with other mental illnesses, like unipolar depression, schizophrenia or ADHD, bipolar disorder, especially bipolar II, remains underdiagnosed.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cPeople with bipolar II often come in not because they&#8217;re hypomanic \u2014 in fact, that doesn&#8217;t seem like a problem to them \u2014 but rather because they&#8217;re depressed,\u201d says Kimmel. \u201cWhat the challenge is is that the doctor doesn&#8217;t know if they&#8217;re depressed because they have bipolar II or whether they have the more classic major depressive disorder. The depressive periods look identical.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In fact, <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC5936046\/\"><span style=\"font-weight: 400;\">nearly 69 percent of patients<\/span><\/a><span style=\"font-weight: 400;\"> with bipolar disorder are misdiagnosed initially. And <a href=\"https:\/\/www.frontiersin.org\/articles\/10.3389\/fpsyg.2020.482715\/full#B23\">one study<\/a> reports it typically takes five to 10 years to receive a correct bipolar diagnosis after the initial request for help.<\/span><span style=\"font-weight: 400;\"> This quickly enters dangerous territory.\u00a0<\/span><\/p>\n<hr class=\"squiggle\" \/>\n<h3>Danger, danger<\/h3>\n<p><span style=\"font-weight: 400;\">When I realized I had bipolar, I was also scared. <\/span><span style=\"font-weight: 400;\">Of all the mental illnesses, bipolar disorder is one of the most fatal. If you have it, you\u2019re <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3767989\/#:~:text=Bipolar%20Disorder%20(BD)%20is%20associated,rate%20in%20the%20general%20population%20(.\"><span style=\"font-weight: 400;\">28 times<\/span><\/a><span style=\"font-weight: 400;\"> more likely to die by suicide than the general population.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201c<\/span><span style=\"font-weight: 400;\">Depression is the dominant pole of bipolar disorder, with bipolar individuals spending on average three times as much time depressed as manic\/hypomanic,\u201d says Dr. Michael Gitlin, director of the Mood Disorders Clinic at UCLA. Kimmel reiterates that bipolar depression can be incredibly difficult to treat. \u201cIt\u2019s not peachy,\u201d he adds.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Misdiagnoses can be dangerous because the treatments for unipolar and bipolar depression are fundamentally different. While both can be treated with psychotherapy, typical depression is often treated with selective serotonin reuptake inhibitors (SSRIs), which can actually worsen manic\/hypomanic episodes or create mixed episodes (a combination of depression and mania) in people with bipolar disorder.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">To treat bipolar disorder, psychiatrists often prescribe a mix of <\/span><a href=\"https:\/\/www.camh.ca\/en\/health-info\/mental-illness-and-addiction-index\/mood-stabilizing-medication\"><span style=\"font-weight: 400;\">mood stabilizers<\/span><\/a><span style=\"font-weight: 400;\">, to reduce mood swings and prevent relapses into depression or mania, and <\/span><a href=\"https:\/\/www.webmd.com\/bipolar-disorder\/guide\/antipsychotic-medication\"><span style=\"font-weight: 400;\">antipsychotics<\/span><\/a><span style=\"font-weight: 400;\">, which can help control delusional thoughts in people with bipolar I. At times, a psychiatrist might add an <a href=\"https:\/\/www.zocdoc.com\/blog\/5-people-on-tapering-off-antidepressants\/\">antidepressant<\/a> into the mix, but more for someone living with bipolar II than I. It can take some time to find the right treatment; <\/span><a href=\"https:\/\/www.prnewswire.com\/news-releases\/new-national-survey-of-people-living-with-bipolar-i-disorder-provides-insights-into-disease-related-challenges-and-the-treatment-journey-301455122.html\"><span style=\"font-weight: 400;\">in one survey<\/span><\/a><span style=\"font-weight: 400;\"> of people with Bipolar I, 54 percent had tried 5 or more different medications to find the <a href=\"https:\/\/www.zocdoc.com\/blog\/5-people-on-taking-ketamine-for-depression\/\">right treatment plan<\/a>.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">I personally was prescribed a mood stabilizer, to go along with my antidepressant, in December 2021. Luckily, I haven\u2019t experienced side effects, and it\u2019s drastically changed my life for the better. Before, I lived on a roller coaster that I couldn\u2019t always control; now I\u2019m able to roll with the punches more. I\u2019m so grateful that I have the resources to get the treatment I need.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Due to stigma, funding issues and misdiagnoses, <\/span><a href=\"https:\/\/www.healthline.com\/health\/bipolar-disorder\/history-bipolar\"><span style=\"font-weight: 400;\">fewer than 40 percent of people<\/span><\/a><span style=\"font-weight: 400;\"> with bipolar disorder receive \u201cminimally adequate treatment.\u201d <\/span><span style=\"font-weight: 400;\">Yet silence about bipolar disorder only prevents those living with it from enjoying the lives they deserve \u2014 and can, with already available help, have.<\/span><\/p>\n<hr \/>\n<h1 class=\"p1\"><span class=\"s1\">Ready to book a doctor\u2019s appointment?
