Today, most of us go to multiple doctors a year.\u00a0<\/span><span style=\"font-weight: 400;\"> Though these advances in modern medicine have helped us better manage and live with certain health conditions, it all requires greater care coordination.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Communication between providers is critical because they can make sure there are no gaps in a patient&#8217;s health plan. For <\/span><span style=\"font-weight: 400;\">instance, a dentist may work with a surgeon to resolve tooth and gum issues. A primary care physician, cardiologist and endocrinologist may work with an individual with heart disease and diabetes.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The better the care coordination, the better the health outcomes. <a href=\"https:\/\/heart.bmj.com\/content\/heartjnl\/102\/1\/50.full.pdf\">Research<\/a> shows that patients who receive care that&#8217;s coordinated by nurses experience lower blood pressure and cholesterol.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Although coordinated care is effective and important, unfortunately, research shows it\u2019s not the norm. <\/span><span style=\"font-weight: 400;\">\u00a0<a href=\"https:\/\/catalyst.nejm.org\/doi\/full\/10.1056\/CAT.18.0291\">One survey<\/a> found that only 7 percent of patients defined their care as fully coordinated. <\/span><\/p>\n<p><span style=\"font-weight: 400;\">Health care providers bear responsibility for care coordination, but patients can also be empowered to take steps to ensure they receive the most comprehensive care from multiple professionals.\u00a0<\/span><\/p>\n<hr \/>\n<h3>Come to appointments well-prepared<\/h3>\n<p><span style=\"font-weight: 400;\">When working with multiple providers, ensure you\u2019re getting the most out of each appointment. Taking time before each appointment to compile information can ensure everyone is on the same page. Before your next check-up, be sure to gather:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Your insurance information<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Payment\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">A list of questions<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Notes about medical concerns<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Mental health outlook<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Physical changes you are experiencing<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">An updated list of medications, including dosage, the reason for prescription and frequency<\/span><\/li>\n<\/ul>\n<h3>Stay in contact with your healthcare team<\/h3>\n<p><a href=\"https:\/\/www.zocdoc.com\/\"><span style=\"font-weight: 400;\">Keeping up with your appointments<\/span><\/a><span style=\"font-weight: 400;\"> and providers can be challenging.\u00a0<\/span><span style=\"font-weight: 400;\">However busy you may be, it\u2019s essential to prioritize your health and stay up-to-date with appointments \u2014 it will help enable you to show up for your job and family in the long run. At these appointments, you\u2019ll want to make sure your providers have the most updated medical history, including other specialists you see.<\/span><\/p>\n<h3>Keep track of providers\u2019 names and contact information<\/h3>\n<p><span style=\"font-weight: 400;\">At appointments, you may be asked to provide a list of other specialists you are seeing. For example, an ob-gyn may ask you who your primary care physician is. Keep a written or running list in the notes app on your phone of your providers\u2019 <\/span><a href=\"https:\/\/www.zocdoc.com\/about\/question\/where-can-i-find-the-doctors-phone-number\"><span style=\"font-weight: 400;\">names, addresses and phone numbers<\/span><\/a><span style=\"font-weight: 400;\">.<\/span><\/p>\n<h3>Access your medical records<\/h3>\n<p><span style=\"font-weight: 400;\">You have a<\/span><a href=\"https:\/\/www.zocdoc.com\/blog\/how-to-request-your-medical-records\/\"><span style=\"font-weight: 400;\"> right to access<\/span><\/a><span style=\"font-weight: 400;\"> hard or digital copies of your medical records. You can simply call your provider, and they have no more than 30 days to provide you with your records (<\/span><span style=\"font-weight: 400;\">this maximum timeframe varies by state<\/span><span style=\"font-weight: 400;\">). Some providers charge a<\/span><a href=\"https:\/\/www.hhs.gov\/hipaa\/for-individuals\/medical-records\/index.html\"><span style=\"font-weight: 400;\"> \u201creasonable\u201d fee.<\/span><\/a><\/p>\n<p><span style=\"font-weight: 400;\">You also may be able to access test results and other records through digital patient portals or apps. Speak with your provider about ways to access your medical records to ensure each professional you are working with has the most updated information.<\/span><\/p>\n<h3>Bring recent test results and notes<\/h3>\n<p><span style=\"font-weight: 400;\">If one doctor sent you for bloodwork or an X-Ray, be sure to bring those results to discuss with other providers. You want your doctors to be able to prescribe you medications and other treatments with full knowledge of your medical background.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Bring discharge notes if you are visiting a doctor after being in the hospital. These notes will give your doctor valuable information on diagnoses and treatment.\u00a0<\/span><\/p>\n<h3>Keep a list of your medical history<\/h3>\n<p><span style=\"font-weight: 400;\">It can be challenging to remember everything about your health or recite it on demand. To ensure each provider has the most current information, keep a running list on your phone or a piece of paper with details on your medical status, including:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Medications<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Surgeries and office procedures and dates<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Family medical history, such as heart conditions<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Allergies<\/span><\/li>\n<\/ul>\n<h3>Allow providers to refer you to other professionals<\/h3>\n<p><span style=\"font-weight: 400;\">It\u2019s common for healthcare providers to refer you to others. The medical field has expanded, and certain professionals may have greater expertise in an area. For example, a <\/span><a href=\"https:\/\/www.zocdoc.com\/dentists\"><span style=\"font-weight: 400;\">dentist may refer you to a dental surgeon.<\/span><\/a><\/p>\n<p><span style=\"font-weight: 400;\">Though making a new appointment with a different professional may feel overwhelming, it\u2019s usually to your benefit. Doctors often refer patients to people they have worked with before and trust. This familiarity can enhance communication between the two.\u00a0<\/span><\/p>\n<h3>Speak up about your concerns<\/h3>\n<p><span style=\"font-weight: 400;\">If you\u2019re concerned about your health, such as the symptoms you\u2019re experiencing, don\u2019t be afraid to speak up. Though the conversation may be difficult or you may feel embarrassed by the symptom, your care team is there to help you be as healthy as possible. This step also applies if you\u2019re concerned about communications between providers.\u00a0<\/span><\/p>\n<h3>Write it down<\/h3>\n<p><span style=\"font-weight: 400;\">Discussing concerns can be nerve-wracking, and you may forget some of them if you are nervous. Jotting down what you want to say can help you remember it and ensure you give your provider all of the information you have.<\/span><\/p>\n<h3>Practice<\/h3>\n<p><span style=\"font-weight: 400;\">If you\u2019re nervous about talking with your provider, try practicing. Try enlisting a trusted friend to practice with or read what you have written in front of a mirror.<\/span><\/p>\n<h3>Explain yourself thoroughly<\/h3>\n<p><span style=\"font-weight: 400;\">Give your doctor the clearest picture of what you are experiencing. Let them know the symptoms, when they started and how long they\u2019ve lasted. The more data and information you can give a doctor, the more prescriptive they can be.\u00a0<\/span><\/p>\n<h3>Be honest<\/h3>\n<p><span style=\"font-weight: 400;\">Refrain from downplaying the situation. You may not display symptoms you are experiencing in the office, so you\u2019ll need to describe them to your doctor. Remember, you are your own best advocate.<\/span><\/p>\n<h3>Listen to what your provider has to say<\/h3>\n<p><span style=\"font-weight: 400;\">Good communicators are also good listeners. Active listening can help you ascertain information and better evaluate it.<\/span><\/p>\n<h3>Take notes<\/h3>\n<p><span style=\"font-weight: 400;\">Sometimes, it can feel like you\u2019re getting a ton of information thrown at you at once.
