Jonathan Lubecky returned from serving in the war in Iraq with debilitating post-traumatic stress disorder. Fourth of July fireworks in his neighborhood triggered terror so profound, he locked himself in a closet, dressed in full body armor.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Gripped by suicidal thoughts and desperate for help, in 2014, Lubecky enrolled in a clinical trial: supervised doses of <\/span><a href=\"https:\/\/psychedelics.berkeley.edu\/substance\/mdma\/\"><span style=\"font-weight: 400;\">MDMA<\/span><\/a><span style=\"font-weight: 400;\">, also known as ecstasy or Molly, which produces a range of euphoric feelings. He went through three sessions, in which a medical provider guided him through an MDMA experience. Lubecky\u2019s suicidal thoughts abated for the first time in eight years.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">He shares his story in a new Netflix documentary, <\/span><a href=\"https:\/\/www.nytimes.com\/2022\/07\/15\/arts\/television\/how-to-change-your-mind-netflix-michael-pollan.html\"><i><span style=\"font-weight: 400;\">How to Change Your Mind<\/span><\/i><\/a><i><span style=\"font-weight: 400;\">. <\/span><\/i><span style=\"font-weight: 400;\">It\u2019s based on a book by journalist Michael Pollan which spotlights psychedelics in medical and mental health treatment. Certain plant-derived substances have attracted renewed interest in recent years and shown promise in alleviating symptoms of PTSD, depression, anxiety, addiction and other mental health disorders.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">For decades, psychedelics \u2014 including psilocybin (\u201cmagic mushrooms\u201d), LSD, MDMA, ketamine, mescaline and others \u2014 were classified as Schedule I illegal drugs with no accepted medical use. But in 2019, the Food and Drug Administration approved a <\/span><a href=\"https:\/\/www.fda.gov\/news-events\/press-announcements\/fda-approves-new-nasal-spray-medication-treatment-resistant-depression-available-only-certified\"><span style=\"font-weight: 400;\">ketamine-based depression medication<\/span><\/a><span style=\"font-weight: 400;\">; it may soon do the same for other psychedelic treatments. In 2020, a <\/span><a href=\"https:\/\/www.forbes.com\/sites\/chrisroberts\/2020\/11\/04\/oregon-legalizes-psilocybin-mushrooms-and-decriminalizes-all-drugs\/?sh=6cc078e4b51d\"><span style=\"font-weight: 400;\">ballot measure in Oregon<\/span><\/a><span style=\"font-weight: 400;\"> legalized psilocybin for therapeutic use.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">This shift has many people curious about psychedelics and their medical uses. Below, we dive into what to expect from the field of psychedelic therapy over the coming years.\u00a0<\/span><\/p>\n<hr \/>\n<h3>An ancient tradition with new scientific implications<\/h3>\n<p><span style=\"font-weight: 400;\">Humans have used consciousness-altering plants for spiritual and medicinal reasons for thousands of years. Some North and Central American indigenous peoples use <\/span><a href=\"https:\/\/www.sciencedirect.com\/topics\/neuroscience\/mescaline\"><span style=\"font-weight: 400;\">peyote-derived mescaline<\/span><\/a><span style=\"font-weight: 400;\"> in religious ceremonies dating back 8,000 years, while numerous Siberian tribes have ingested the psychedelic mushroom <\/span><a href=\"https:\/\/www.mdpi.com\/2673-8392\/1\/3\/69\/htm\"><span style=\"font-weight: 400;\">fly agaric<\/span><\/a><span style=\"font-weight: 400;\"> for sacred purposes for millennia.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In the early 20th century, Western scientists began researching psychedelics\u2019 effect on the human mind, studying both naturally occurring substances like mescaline and <\/span><a href=\"https:\/\/www.emcdda.europa.eu\/publications\/drug-profiles\/lsd_en\"><span style=\"font-weight: 400;\">lab-created drugs like LSD<\/span><\/a><span style=\"font-weight: 400;\"> (lysergic acid diethylamide). Early studies by psychiatrists showed that such mind-altering drugs had enormous potential for treating disorders such as addiction, depression and anxiety.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">But during the social and political upheaval of the 1960s, psychedelics \u201cescaped the lab,\u201d as Pollan puts it. Young people especially experimented with them recreationally, which caused a backlash of public concern about the drugs\u2019 potential dangers. As the \u201cSwinging \u201860s\u201d gave way to the \u201claw and order\u201d 1970s, the US government classified all psychedelics as <\/span><a href=\"https:\/\/maps.org\/news\/media\/medical-daily-the-war-on-drugs-may-have-misrepresented-psychedelics-here-s-why-that-matters\/\"><span style=\"font-weight: 400;\">Schedule I illegal drugs<\/span><\/a><span style=\"font-weight: 400;\">, part of the larger \u201cwar on drugs.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Only recently have clinical trials revisited the promising findings of earlier decades. <\/span><a href=\"https:\/\/www.frontiersin.org\/articles\/10.3389\/fpsyt.2022.831092\/full\"><span style=\"font-weight: 400;\">New studies<\/span><\/a><span style=\"font-weight: 400;\"> found that when administered in a clinical setting with psychotherapeutic support, <a href=\"https:\/\/www.zocdoc.com\/blog\/taking-mushrooms-seriously\/\">psychedelics<\/a> can produce rapid and long-lasting improvement for patients with serious depression, anxiety, obsessive-compulsive disorder, PTSD, alcohol addiction and smoking. Psilocybin has also been found to <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC5367557\/\"><span style=\"font-weight: 400;\">alleviate anxiety and depression<\/span><\/a><span style=\"font-weight: 400;\"> in patients with life-threatening cancer.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The University of California, Berkeley, is one of several institutions worldwide that share research, best practices and training tools for use of psychedelics in spiritual and healthcare settings.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cAt this moment, psychedelics are expanding into many different aspects of our culture and our healthcare system,\u201d explains Rev. Moana Meadow, a healthcare chaplain and a member of the Psychedelic Facilitation Certificate Program at <\/span><a href=\"https:\/\/psychedelics.berkeley.edu\/\"><span style=\"font-weight: 400;\">Berkeley\u2019s Center for the Science of Psychedelics<\/span><\/a><span style=\"font-weight: 400;\">.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The center trains healthcare providers and religious and spiritual care professionals in facilitating psychedelic experiences for spiritual and psychotherapeutic care safely and effectively.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Given the promising results of clinical trials, government agencies like the FDA are considering <\/span><a href=\"https:\/\/www.amjmed.com\/article\/S0002-9343(21)00521-0\/fulltext\"><span style=\"font-weight: 400;\">legalizing more psychedelics for medical purposes<\/span><\/a><span style=\"font-weight: 400;\">.<\/span><\/p>\n<hr \/>\n<h3>The current landscape<\/h3>\n<p><span style=\"font-weight: 400;\">At the moment, ketamine is the only FDA-approved psychedelic, used off-label to treat depression and suicidal thoughts as a <\/span><a href=\"https:\/\/health.clevelandclinic.org\/a-new-ketamine-nasal-spray-has-been-approved-for-depression-but-its-not-for-everyone\/\"><span style=\"font-weight: 400;\">nasal spray called Spravato<\/span><\/a><span style=\"font-weight: 400;\">. But MDMA could be close behind: After encouraging results, clinical trials <\/span><a href=\"https:\/\/www.nytimes.com\/2022\/01\/05\/well\/psychedelic-drugs-mental-health-therapy.html\"><span style=\"font-weight: 400;\">moved to Phase 3<\/span><\/a><span style=\"font-weight: 400;\"> in 2021, and this summer, the Biden administration <\/span><a href=\"https:\/\/maps.org\/2022\/07\/27\/statement-biden-administration-preparing-for-potential-fda-approval-of-mdma-assisted-therapy-for-ptsd\/\"><span style=\"font-weight: 400;\">released a letter<\/span><\/a><span style=\"font-weight: 400;\"> saying it expects the FDA to approve MDMA for PTSD treatment as soon as 2023.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Meanwhile, <\/span><a href=\"https:\/\/www.clinicaltrialsarena.com\/analysis\/psychedelics-psychedelic-drug-trials-aim-to-break-new-ground-in-mental-health\/\"><span style=\"font-weight: 400;\">dozens of clinical trials<\/span><\/a><span style=\"font-weight: 400;\"> across the US are studying therapeutic uses for LSD, psilocybin and other psychedelics. In Oregon, the first state to legalize psilocybin for therapeutic use, agencies will roll out treatment services <\/span><a href=\"https:\/\/www.prnewswire.com\/news-releases\/oregon-to-become-the-first-state-to-legally-roll-out-psilocybin-301535047.html\"><span style=\"font-weight: 400;\">next year<\/span><\/a><span style=\"font-weight: 400;\">. Researchers and regulators nationwide will be watching closely to see how such a framework might work elsewhere.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">For now, patients interested in these treatments must enroll in clinical trials, which provide a safe, supervised psychedelic therapy experience. Trial volunteers are carefully screened; they must meet the criteria of the study, which could mean being a certain age, having a certain health diagnosis or a specific life history.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">There are also a few <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC8905125\/\"><span style=\"font-weight: 400;\">health risks<\/span><\/a><span style=\"font-weight: 400;\"> that exclude certain populations from psychedelic clinical trials, explains Matthew Johnson, a professor of psychiatry and behavioral science at Johns Hopkins University School of Medicine.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Taking psychedelics can raise blood pressure and heart rate, and in rare cases can induce psychotic symptoms in people with a predisposition for psychotic illnesses like schizophrenia. Therefore, says Johnson, \u201cit is important to exclude folks with severe heart disease and folks who are suffering from psychotic disorders or those who show a predisposition to such disorders.\u201d\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Extensive research has found that in clinical settings where participants are carefully screened and supervised by professionals, psychedelic therapy <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4813425\/#:~:text=Although%20there%20is%20a%20general,LSD%2C%20psilocybin%2C%20or%20mescaline.\"><span style=\"font-weight: 400;\">is very safe<\/span><\/a><span style=\"font-weight: 400;\">.\u00a0<\/span><\/p>\n<hr \/>\n<h3>Future of psychedelic therapy<\/h3>\n<p><span style=\"font-weight: 400;\">If the FDA approves more psychedelics for use in medical care, more people will be eligible for this kind of treatment. Yet psychedelics aren\u2019t your typical pharmaceutical, which means we probably won\u2019t see doctors simply write a script for an acid trip.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Rather, mental health providers and medical professionals will work with patients to determine whether a specific psychedelic therapy treatment is right for them, and that treatment will be delivered in a supervised setting. It could be a one-time experience or an ongoing treatment; some clinical trials are studying the <\/span><a href=\"https:\/\/trialsjournal.biomedcentral.com\/articles\/10.1186\/s13063-021-05243-3\"><span style=\"font-weight: 400;\">potential benefits of microdosing<\/span><\/a><span style=\"font-weight: 400;\"> (taking very low levels of a drug at regular intervals) psilocybin or LSD for patients with depression.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Oregon\u2019s forthcoming <\/span><a href=\"https:\/\/www.oregon.gov\/oha\/PH\/PREVENTIONWELLNESS\/Pages\/Oregon-Psilocybin-Services.aspx\"><span style=\"font-weight: 400;\">psilocybin service<\/span><\/a><span style=\"font-weight: 400;\"> is managed by the Oregon Health Authority, and will be available in licensed facilities, under trained supervision, to adults over age 21.\u00a0<\/span><span style=\"font-weight: 400;\">Other states are <\/span><a href=\"https:\/\/www.pewtrusts.org\/en\/research-and-analysis\/blogs\/stateline\/2022\/07\/15\/more-states-may-legalize-psychedelic-mushrooms\"><span style=\"font-weight: 400;\">considering legalizing psilocybin<\/span><\/a><span style=\"font-weight: 400;\">, including California, Colorado and New Jersey. If such measures pass, each state may adopt its own framework for legal use of psychedelics.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Safety protocols for providers and facilitators will also be critical, something that\u2019s already built into clinical trials, says Johnson. \u201cIt would be a specialty treatment, not something a clinician would do on the side,\u201d he says. \u201cIt requires specialized training on how to conduct this treatment.\u201d\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Proper facilitation includes patient screening, preparation, supervision and follow-up care \u2014 \u201cnot \u2018Take two and call me in the morning,\u2019\u201d Johnson adds.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">There is still a lot to learn about psychedelic treatment in therapeutic settings, but researchers hope this field can provide real and lasting benefits. After his transformational experience with clinically supervised doses of ecstasy, Sgt. Lubecky became an advocate for treating PTSD in veterans with MDMA. He\u2019s spoken at police conventions and <\/span><a href=\"https:\/\/theintercept.com\/2022\/07\/14\/ptsd-psychedelic-therapy-research-congress\/\"><span style=\"font-weight: 400;\">submitted testimony<\/span><\/a><span style=\"font-weight: 400;\"> for congressional hearings on drug law reform.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cMy story is the same story as millions of veterans,\u201d Lubecky says in the Netflix<\/span> <span style=\"font-weight: 400;\">documentary. \u201cWhere my story becomes unique is the fact that I took MDMA three times.
