Now What?"},"content":{"rendered":"<p><span style=\"font-weight: 400;\">So you\u2019ve just been diagnosed with genital herpes. First of all \u2014 welcome to the club. If you\u2019re anything like me, when my gynecologist gave me the news two years ago, you may be frantically googling your symptoms and shedding a few tears (okay, a lot of tears), worrying about how your next love interest will react.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Take a breath. You\u2019re not alone.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In fact, <\/span><a href=\"https:\/\/www.cdc.gov\/std\/herpes\/stdfact-herpes-detailed.htm\"><span style=\"font-weight: 400;\">around 527,000 people<\/span><\/a><span style=\"font-weight: 400;\"> in the U.S. get the diagnosis every year. Regardless of whether you\u2019re experiencing symptoms, lack of education and scant media representation makes it challenging to navigate next steps. Here\u2019s my advice on what to do after a positive diagnosis.\u00a0<\/span><\/p>\n<hr \/>\n<h2>1. Educate yourself.<\/h2>\n<p><span style=\"font-weight: 400;\">Since <\/span><a href=\"https:\/\/worldpopulationreview.com\/state-rankings\/abstinence-only-education-states\"><span style=\"font-weight: 400;\">19 states<\/span><\/a><span style=\"font-weight: 400;\"> practice abstinence-only sex education, it\u2019s possible that your herpes knowledge base consists only of fear-mongering slideshows and stories of painful blisters. It\u2019s time to replace these messages with basic herpes facts.\u00a0<\/span><\/p>\n<h3><i>There are two strains.<\/i><\/h3>\n<p><span style=\"font-weight: 400;\">You may have heard about \u201cabove the belt\u201d (HSV-1) and \u201cbelow the belt\u201d (HSV-2) herpes diagnoses. The first mostly manifests as cold sores, while the second is typically seen as genital sores. Increasingly, however, medical professionals are moving away from those simplifications, says<\/span> <a href=\"https:\/\/leelabvirus.host\/team\"><span style=\"font-weight: 400;\">Dr. Jillian Carmichael<\/span><\/a><span style=\"font-weight: 400;\">, a virologist at the Icahn School of Medicine at Mount Sinai in New York<\/span><span style=\"font-weight: 400;\">. Each strain requires different treatments, but we can\u2019t be sure that location alone tells us which virus is responsible for an infection.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">As an example, <\/span><span style=\"font-weight: 400;\">HSV-2 is spread via genital-to-genital contact. The Centers for Disease Control and Prevention (CDC) <\/span><a href=\"https:\/\/www.cdc.gov\/std\/herpes\/stdfact-herpes.htm#:~:text=Yes.,are%20due%20to%20HSV%2D1.\"><span style=\"font-weight: 400;\">notes<\/span><\/a><span style=\"font-weight: 400;\"> its cold sore counterpart, HSV-1, can be transmitted through oral sex \u2014 and can cause genital herpes. Your doctor may order an <\/span><a href=\"https:\/\/www.webmd.com\/genital-herpes\/what-is-herpes-simplex-virus-antibodies-test\"><span style=\"font-weight: 400;\">STI blood test<\/span><\/a><span style=\"font-weight: 400;\"> to confirm the infection is present, but a <\/span><span style=\"font-weight: 400;\">swab test of an outbreak lesion is the gold standard for an HSV diagnosis.<\/span><\/p>\n<h3><em>Many people have herpes and don\u2019t know.\u00a0<\/em><\/h3>\n<p><span style=\"font-weight: 400;\">When educating his patients, Dr. Kevin Ard, director of the <\/span><a href=\"https:\/\/www.massgeneral.org\/medicine\/infectious-diseases\/treatments-and-programs\/sexual-health-program\"><span style=\"font-weight: 400;\">Sexual Health Clinic at Massachusetts General Hospital<\/span><\/a><span style=\"font-weight: 400;\"> in Boston, emphasizes how common that herpes is. \u201cThe majority of Americans have HSV-1, and about 1 in 5 Americans have HSV-2,\u201d he says. \u201cIf you look at any crowd of people, most people have one form of HSV.\u201d\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">T<\/span><span style=\"font-weight: 400;\">he CDC <\/span><a href=\"https:\/\/www.cdc.gov\/std\/herpes\/screening.htm\"><span style=\"font-weight: 400;\">does not recommend<\/span><\/a><span style=\"font-weight: 400;\"> providers test patients for herpes if they don\u2019t have symptoms, so it isn\u2019t included in most STI panels. This lack of routine HSV screening is a big part of why it\u2019s so common.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201c<\/span><span style=\"font-weight: 400;\">While the risk of HSV transmission is higher with visible lesions, asymptomatic transmission does occur,\u201d <\/span><span style=\"font-weight: 400;\">Carmichael says.<\/span><span style=\"font-weight: 400;\"> \u201cA person can transmit the virus to someone else without ever knowing they were infected<\/span><span style=\"font-weight: 400;\">.\u201d\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">For those who end up showing symptoms (there\u2019s no way yet to predict who will or won\u2019t), the first outbreak is usually the roughest, often lasting a few weeks. However, over time, the body becomes accustomed to the infection, and outbreaks become milder and less frequent. \u201cThere\u2019s a lot of variation in how often people have outbreaks,\u201d says Ard. \u201cSome people never have an outbreak again after the first one.\u201d\u00a0<\/span><\/p>\n<h3><i>There\u2019s no cure for herpes\u2026 yet.<\/i><\/h3>\n<p><span style=\"font-weight: 400;\">While researchers have floated potential vaccines, nothing has been FDA-approved yet. HSV-1 and HSV-2 are in their own category as <a href=\"https:\/\/www.zocdoc.com\/blog\/can-you-get-stis-without-having-sex\/\">STIs<\/a>, but they\u2019re just two of the <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK8157\/#:~:text=Herpesviruses%20are%20divided%20into%20three,and%207%2C%20with%20a%20long\"><span style=\"font-weight: 400;\">many herpesviruses out there<\/span><\/a><span style=\"font-weight: 400;\">. For instance, chickenpox and shingles are caused by a herpesvirus, varicella-zoster.This family of viruses also acts similarly in the body.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201c<\/span><span style=\"font-weight: 400;\">Once you are infected with a herpesvirus, you\u2019re infected for life,\u201d Carmichael says. \u201cHSV is very sneaky and establishes latency, aka a permanent infection, in neuronal (nervous system) cells.\u201d<\/span><\/p>\n<hr \/>\n<h2>2. Affirm your self-worth.<\/h2>\n<p><span style=\"font-weight: 400;\">Even in increasingly sex-positive 2022, a <\/span><a href=\"https:\/\/www.nytimes.com\/2019\/08\/13\/style\/sti-stigma-sexual-transmitted-infections.html\"><span style=\"font-weight: 400;\">shameful stigma<\/span><\/a><span style=\"font-weight: 400;\"> remains around STIs. Gynecologist and author Dr. Jen Gunter <\/span><a href=\"https:\/\/www.nytimes.com\/2019\/08\/13\/style\/sti-stigma-sexual-transmitted-infections.html\"><span style=\"font-weight: 400;\">called<\/span><\/a><span style=\"font-weight: 400;\"> them \u201cone of the last taboos.\u201d\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">If you notice yourself feeling shame, disgust or other unpleasant emotions post-diagnosis, it can help to remember that this stigma has been drilled into you from the outside for years. It means nothing about your own value.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Carmichael recommends talking to a therapist or joining herpes-positive communities online. I have also utilized other resources, like writer <\/span><a href=\"https:\/\/youtu.be\/YcIl-hclrLI\"><span style=\"font-weight: 400;\">Ella Dawson\u2019s 2016 TED Talk<\/span><\/a><span style=\"font-weight: 400;\"> \u201cSTIs aren&#8217;t a consequence. They&#8217;re inevitable.\u201d\u00a0<\/span><span style=\"font-weight: 400;\">I\u2019ve also found comfort in Instagram accounts like <\/span><a href=\"https:\/\/www.instagram.com\/courtneybrame_\/?hl=en\"><span style=\"font-weight: 400;\">Courtney Brame\u2019s<\/span><\/a><span style=\"font-weight: 400;\">; he runs the <\/span><span style=\"font-weight: 400;\">nonprofit <\/span><a href=\"https:\/\/www.spfpp.org\/\"><span style=\"font-weight: 400;\">Something Positive for Positive People<\/span><\/a><span style=\"font-weight: 400;\">, which helps normalize the virus and provides mental health support.\u00a0<\/span><\/p>\n<hr \/>\n<h2>3. Practice disclosing.<\/h2>\n<p><span style=\"font-weight: 400;\">If you\u2019re actively dating or seeking future sexual partners, doctors encourage you to disclose your status with them.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">This can seem intimidating, especially if you\u2019re worried about rejection. As with other fear-inducing activities, it helps to practice. Whether that means disclosing to nonsexual partners like friends or family, to strangers in a support group or even to your dog as rehearsal (like me), it gets less scary over time.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Discussions of STIs shouldn\u2019t be a one-way street. You can invite dialogue by simply stating, \u201cLet\u2019s talk about STI statuses. I have tested positive for genital herpes. Here\u2019s what it means for sex. What about you?\u201d\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">By having this discussion, you are demonstrating care for your partners and their right to informed consent. Future partners are lucky to have you. You\u2019ll also probably find that how you open up matters; if you present the news calmly, people will typically mirror you.\u00a0<\/span><\/p>\n<hr \/>\n<h2>4. Develop a herpes management plan.<\/h2>\n<p><span style=\"font-weight: 400;\">Although herpes is not curable, it is manageable. Over time, it could have as much effect on your life as the occasional pimple.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Typically, when you\u2019re first diagnosed, you might be prescribed <\/span><a href=\"https:\/\/www.webmd.com\/drugs\/2\/drug-6279\/valacyclovir-oral\/details\"><span style=\"font-weight: 400;\">valacyclovir (Valtrex)<\/span><\/a><span style=\"font-weight: 400;\">,<\/span><span style=\"font-weight: 400;\"> which suppresses the virus. While some people take medication every day to prevent outbreaks and reduce transmission risk, others take it as needed to stop an existing outbreak.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Ard makes sure his newly diagnosed patients get access to valacyclovir immediately. \u201c<\/span><span style=\"font-weight: 400;\">The earlier they take it, the better,\u201d he says. \u201cIf they find they\u2019re having to use that outbreak-focused medicine very often, or if severe outbreaks interfere with their life, we talk about using a lower dose of medication consistently to prevent outbreaks from happening.\u201d\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">People who are immunocompromised are more likely to encounter frequent outbreaks and may benefit from daily, preventive medication. If you\u2019re pregnant, a doctor can help you come up with a plan to avoid any risks to your baby.\u00a0<\/span><\/p>\n<hr \/>\n<p><span style=\"font-weight: 400;\">As you move forward after your diagnosis, remember that you aren\u2019t alone. As I\u2019ve become more open about my herpes status, I\u2019ve connected more with other herpes-positive folks, educated others and slowly chipped away at my own internalized stigma.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Herpes is common. It\u2019s manageable. And you\u2019re still the person and partner you were before you contracted it.
