Transgender patients face a range of <\/span><a href=\"https:\/\/www.americanprogress.org\/article\/protecting-advancing-health-care-transgender-adult-communities\/\"><span style=\"font-weight: 400;\">healthcare challenges<\/span><\/a><span style=\"font-weight: 400;\">, many of which stem from the <\/span><a href=\"https:\/\/www.lgbtqandall.com\/causes-of-minority-stress-in-transgender-individuals\/\"><span style=\"font-weight: 400;\">stress of discrimination<\/span><\/a><span style=\"font-weight: 400;\">. These health issues, plus a high demand for transition-related care, should make us frequent flyers in doctor\u2019s offices.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">However, these spaces can feel unsafe. Since medical providers can direct the course of our health care, disclosing our status as transgender to a discriminatory provider places us at risk of information breach and personal harm. In <\/span><a href=\"https:\/\/www.thetaskforce.org\/new-report-reveals-rampant-discrimination-against-transgender-people-by-health-providers-high-hiv-rates-and-widespread-lack-of-access-to-necessary-care-2\/\"><span style=\"font-weight: 400;\">one survey<\/span><\/a><span style=\"font-weight: 400;\">, <\/span><span style=\"font-weight: 400;\">28 percent of trans patients reported experiencing harassment in medical settings.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Even providers who are not intentionally discriminatory but are unaware of the intricacies of transgender healthcare can provide poorer treatment. \u201c<\/span><a href=\"https:\/\/lgbt.foundation\/who-we-help\/women\/our-stories\/lbt-womens-health-week\/ashleigh---diagnostic-overshadowing\"><span style=\"font-weight: 400;\">Trans broken arm syndrome<\/span><\/a><span style=\"font-weight: 400;\">\u201d describes the propensity of providers to misattribute the cause of any ailment in a trans patient to their medical transition. In the survey cited above, half of trans patients reported teaching their doctors how to provide transgender care.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The expectation of mistreatment and a range of other healthcare disparities contributes to <\/span><a href=\"https:\/\/www.transformingfuturespartnership.co.uk\/healthcare\"><span style=\"font-weight: 400;\">health care anxiety<\/span><\/a><span style=\"font-weight: 400;\"> that keeps us from accessing healthcare despite keenly needing it. While there\u2019s been an <\/span><a href=\"https:\/\/www.bu.edu\/sph\/news\/articles\/2022\/transgender-and-nonbinary-identified-health-care-professionals-are-changing-the-exclusionary-health-care-system\/\"><span style=\"font-weight: 400;\">increase in trans-friendly<\/span><\/a><span style=\"font-weight: 400;\"> healthcare providers out there, we still have a long way to go. <\/span><\/p>\n<p><span style=\"font-weight: 400;\">Trans and queer-aligned NGOs are a first port-of-call for trans patients looking for healthcare, and some are taking steps to <\/span><a href=\"https:\/\/www.bizneworleans.com\/lcmc-health-announces-partnerships-to-address-cultural-competency\/\"><span style=\"font-weight: 400;\">partner with medical institutions<\/span><\/a><span style=\"font-weight: 400;\"> for referrals, outreach and provider education.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">For now, I break down what types of information you might want to talk about with your medical provider if you identify as transgender.\u00a0<\/span><\/p>\n<hr \/>\n<h3>Opening up<\/h3>\n<p><span style=\"font-weight: 400;\">There isn\u2019t a single transgender body. Your identity as a trans person is bound up in a complex web of sociocultural, physiological, emotional and political factors that vary from person to person.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Some trans people have obtained sufficient treatment and surgery to be effectively indistinguishable from cisgender people. Others forgo medical transition, whether or not by choice. Still others have<\/span> <span style=\"font-weight: 400;\">intersex conditions (a difference between the external and internal genitals) that can affect their gender identity.<\/span><\/p>\n<p><a href=\"https:\/\/doctomson.co.za\/gp\/\"><span style=\"font-weight: 400;\">Dr. Anastacia Tomson<\/span><\/a><span style=\"font-weight: 400;\">, an LGBTQ+ rights activist and trans woman based in South Africa, says that healthcare anxieties for trans patients \u201c<\/span><span style=\"font-weight: 400;\">can be manifold.\u201d Still, <\/span><span style=\"font-weight: 400;\">if you\u2019re a trans patient meeting with a new doctor, \u201cIdeally, you should always disclose medical information, particularly if you have received medical or surgical interventions.\u201d\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Before you even enter the doctor\u2019s office, however, you\u2019ll often have to navigate billing and insurance. This can be especially challenging for trans people whose documentation does not match their presentation or identity. Sometimes it\u2019s necessary to disclose your transgender status to the doctor\u2019s office to avoid bureaucratic confusion and receive care. This may include presenting assigned sex at birth or a dead name to the office, potentially in the presence of other patients and unrelated staff. At minimum, this can out a patient and be a distressing experience, and at worst can result in <\/span><a href=\"https:\/\/www.nbcnews.com\/nbc-out\/out-health-and-wellness\/nearly-half-trans-people-mistreated-medical-providers-report-finds-rcna1695\"><span style=\"font-weight: 400;\">stigma or mistreatment<\/span><\/a><span style=\"font-weight: 400;\">. This is one reason why the right to change gender markers and identity documents is so important.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Once you\u2019re in the room, you\u2019ll want to talk to your doctor and assess whether they\u2019re a good fit.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">If you don\u2019t feel comfortable being fully honest with your provider, that might mean they\u2019re not the best fit, Tomson advises. Assessing whether doctors are queer-friendly, or a good fit can be fraught with uncertainty. Often, doctors who <\/span><a href=\"https:\/\/www.cuimc.columbia.edu\/news\/pride-and-health-care-why-welcoming-providers-are-essential-lgbtq-health\"><span style=\"font-weight: 400;\">advertise themselves as queer-friendly<\/span><\/a><span style=\"font-weight: 400;\"> become a go-to resource for queer patients. These doctors attract queer patients who feel safe seeing them, and the doctors then develop experience in treating queer communities. Local branches of national nonprofits such as <\/span><a href=\"https:\/\/www.plannedparenthood.org\/get-care\/our-services\"><span style=\"font-weight: 400;\">Planned Parenthood<\/span><\/a><span style=\"font-weight: 400;\"> can also make referrals to known queer-friendly doctors, or provide care themselves. There are also databases of queer-friendly healthcare providers, such as <\/span><a href=\"https:\/\/www.outcarehealth.org\/outlist\/\"><span style=\"font-weight: 400;\">Outlist<\/span><\/a><span style=\"font-weight: 400;\">.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Whatever you do, you shouldn\u2019t lie to your provider. \u201c<\/span><span style=\"font-weight: 400;\">To be fair and get the best outcomes, it is helpful for the medical team caring for a patient to know everything that makes up their body,\u201d says Ilene Corina,<\/span><span style=\"font-weight: 400;\">\u00a0a <a href=\"https:\/\/www.icorina.com\/\">patient safety advocate<\/a> in New York.<\/span><span style=\"font-weight: 400;\">\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Despite the challenges we encounter in healthcare, the benefits of lying rarely outweigh the risks incurred to our health.<\/span><span style=\"font-weight: 400;\"> Let\u2019s get into what parts of your medical history you\u2019ll want to discuss with your doctor to the best of your ability.\u00a0<\/span><\/p>\n<hr \/>\n<h3>Past transition care<span style=\"font-weight: 400;\">\u00a0<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">It\u2019s important to talk about any past transition care, such as hormone replacement therapy, with a new doctor. \u201c<\/span><span style=\"font-weight: 400;\">Having access to more information allows for better clinical decision-making and rational medical care,\u201d <\/span><span style=\"font-weight: 400;\">Tomson says.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">This is also pertinent when switching providers or seeing multiple providers. Tomson notes that it\u2019s helpful to \u201c<\/span><span style=\"font-weight: 400;\">furnish your new provider with as much of the clinical history as possible.\u201d <\/span><span style=\"font-weight: 400;\">\u201c<\/span><span style=\"font-weight: 400;\">This allows for better continuity of care, and avoids unnecessary duplication of investigations or gaps in management of ongoing medical concerns,\u201d she adds.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Being honest about past treatments also prevents certain misdiagnoses. For instance, <\/span><span style=\"font-weight: 400;\">some effects of HRT may present diagnostically as a hormone imbalance in someone who presents as the opposite gender. For example, a masculine-presenting trans woman on feminizing hormone therapy will develop breast tissue. This may be misidentified as <\/span><a href=\"https:\/\/www.mayoclinic.org\/diseases-conditions\/gynecomastia\/symptoms-causes\/syc-20351793#:~:text=Gynecomastia%20(guy%2Dnuh%2Dkoh,or%20both%20breasts%2C%20sometimes%20unevenly.\"><span style=\"font-weight: 400;\">gynecomastia<\/span><\/a><span style=\"font-weight: 400;\">.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Likewise, the effects of masculinizing HRT on a patient read as a woman may be misidentified as an effect of <\/span><a href=\"https:\/\/www.mayoclinic.org\/diseases-conditions\/pcos\/symptoms-causes\/syc-20353439\"><span style=\"font-weight: 400;\">polycystic ovary syndrome<\/span><\/a><span style=\"font-weight: 400;\"> or other hormone disorders.\u00a0<\/span><span style=\"font-weight: 400;\">Disclosing to prevent misdiagnosis or the deployment of unnecessary procedures (pregnancy tests or prostate screening) can save money and ensure quality care.<\/span><\/p>\n<hr \/>\n<h3>Anatomical information<\/h3>\n<p><span style=\"font-weight: 400;\">A range of healthcare risks and conditions affect anatomy independent of gender identity. \u201c<\/span><span style=\"font-weight: 400;\">A trans woman will still need a prostate exam, and a trans man might still get pregnant,\u201d says Corina.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">However, providers <\/span><span style=\"font-weight: 400;\">may not always be aware of a patient\u2019s internal anatomy. They might not ask a trans woman about prostate health, for example, if they\u2019re unaware of the sex they were assigned at birth. It\u2019s often up to the patient to disclose this information in order to get the care they need.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Gender transition is a journey. The road we walk to become our happiest selves is seldom free from uncertainty and risk. Still, positive change is happening. The quality of healthcare for transgender patients improves with each passing year. <\/span><a href=\"https:\/\/www.uchicagomedicine.org\/forefront\/news\/new-uchicago-medicine-clinic-helps-transgender-patients-find-doctors\"><span style=\"font-weight: 400;\">More and more providers receive<\/span><\/a><span style=\"font-weight: 400;\"> better <\/span><a href=\"https:\/\/www.studyinternational.com\/news\/medical-schools-in-the-us-lgbtq\/\"><span style=\"font-weight: 400;\">training on caring for LGBTQ+ bodies<\/span><\/a><span style=\"font-weight: 400;\">.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Hopefully someday, trans patients will be able to confidently talk about their medical history with not just select medical providers but all of them.<\/span><\/p>\n<hr \/>\n<h1 style=\"text-align: center;\">Need a doctor?
