Dr. Adalina Kulbak has been providing exceptional eye-care for families and individuals for over 20 years across the Chicagoland area. She specializes in a broad range of contact lens fittings and has deep experience examining and treating diagnoses such as: cataracts, glaucoma, macular degeneration, diabetic retinopathy and dry eyes. Dr. Kulbak can help patients with more serious eye conditions by prescribing medication, removing foreign bodies, offering surgical referrals and co-managing cataract and lasik surgeries.
A state-licensed eye health care professional, Dr. Kulbak completed her B.S. in Biology and Visual Science from Loyola University of Chicago and PhD from the Illinois College of Optometry in 1993. Dr. Kulbak is also fluent in Russian and Polish.

She very professional. She want to know what you are here for and why. She knew her around the computer and was quick with my exam. I would go back.
It was ok .. besides my kids not sitting still lbs and quik .
First problem: I wasn't in their scheduling database as having an appointment. They treated me as if I was a major inconvenience. Dr. Kulbak seemed annoyed with my presence loudly complained about me to her staff. She performed her services skillfully and answered all of my questions. There was nothing wrong with her skills as a doctor, I was pleased with that. However, the way I was treated, condescendingly, belittling, and as though I was a completely inept person, was unacceptable. Additionally, she asked me the same set of questions over and over, even within seconds of the last time she had asked. This was distressing because literally every question she asked me had been submitted via ZocDoc and also written on the intake forms.
They are very rude and they unprofessionally and I never go back to this place.
One of the rudest physicians I've seen. It was my first appointment with her, and naturally I was trying to tell her information in the past so she can be caught up on what to expdct from my eyeballs. For example I have pretty big eyes so I need a larger diameter for my contacts. As I was trying to tell her this, she kept cutting me off and wasn't understanding what I was saying, so I kept having to repeat myself (which she would have understood if she listened to what I had to say). Once I finally said that I needed a larger diameter, she replied, "I've been doing this for 20 years. I think I know how to do an exam to see if you need a larger size." Later she finds that I do in fact need a larger diameter. Next, I have a specific brand that I'm in love with. She said she didn't have it and didn't believe it was in production because she hadn't heard of it before. I was very persistent in having this brand so after arguing with her for twenty minutes on which brand to write be a script for, the whole time she's giving me sass for not believing this brand exists and shows me a manual she has that doesn't include it, she shows me a flyer for the brands she recommends. And guess what. At the top of the list, there's the brand that I want and that she doesn't believe exists. From being rude in the way she asks me questions to flat out denying what I ask for, I do not recommend her at all.
She was ok. But definitely could tell I was treated differently because I had Medicaid.
This doctor was very rude and it seemed like she just did not want to be there. Everything about this visit was just off. I actually ended up leaving and going to America'S Best and getting glasses because I was so unsatisfied.
Busy holiday visit but overall good service and conversation.
It was far more pricey that I have ever paid. Nearly 3x as much actually. The service was good, she was very fast and I was in and out in about 30 minutes.
I was disappointed with my visit to Dr. Kulbak's office. I felt as though she was not hearing anything I was saying and had no interest in listening. How she handled the procedures themselves and her bedside manner were lacking. I found a new OD.

The doctor had a very bad attitude the moment we walked in the door. We were looking at frames to past the time when she basically ran us off from the wall. I will never go back which is sad for her because I have a big family.

Speaks very loud, no one had to struggle to listen to our conversation. I think everyone in the store knew what insurance I had and that I had to pay for certain services.
Nobody called to confirm my appointment I called the number listed I didn't get an answer
The Doctor is very condescending and made me feel less than. The doctor told me my prescription glasses would be ready in about 6 weeks due to my HMO from the medical card. I waited and its now april 20th and I have yet to receive them and my insurance confirmed the order was reviewed Feb 22nd and the glasses were sent out on Feb 29th and never reported stolen.
I felt at ease with Dr. kulbak and will definitely visit the office again.

My experience as well as my sons (who was the one visiting), was amazing. They were super friendly and explained things very clearly. I'm not familiar with glasses since I've never worn them so it was nice to get information for my sons glasses. They were super helpful with sizing and letting my son try on different pairs. I definitely recommend them and will be brining my younger child there as well.
She was very professional. A little blunt and straight forward, but she did well.
