“Michael has helped me find treatments that have significantly improved my quality of life. He has also helped me decrease some medications which I had been on for many years to lower doses with minimal symptoms. I am noticing improvements in some areas of my health since decreasing the dose (such as lower blood pressure) and my mental health has remained relatively stable throughout the process. I was quite nervous about doing this and I am grateful for his guidance and expert opinion throughout this process.”

Getting to know Michael Perez

I am a board-certified Psychiatric Mental Health Nurse Practitioner (PMHNP) specializing in treating mood, anxiety, cognitive, trauma-related, schizophrenia, and other psychotic disorders. I earned my Master of Science in Nursing with a focus on Psychiatric Mental Health Nursing from the University of California, San Francisco. Prior to this, I obtained a Bachelor of Science in Psychology from the University of California, San Diego, and a Bachelor of Science in Nursing from California State University, Fullerton. I have long held a passion for mental healthcare and have worked with individuals of all ages across various psychiatric settings, including inpatient, outpatient, PHP and IOP programs, mobile healthcare, and research.

What should a new client know about working with you?

My interest in mental health care began during my high school years simply as a fascination with psychology and neuropsychological processes. As I progressed through my education, this grew into a desire to be part of a field that seeks to increase awareness of and decrease stigma surrounding an often-overlooked, but vital, part of health.

What is your typical process for working with clients?

I value taking a collaborative approach with clients to create a thorough, individualized, and evidence-based treatment plan that best meets their specific needs and goals. I am a strong proponent of the philosophy that health is multifactorial, made up of physical, psychological, emotional, social and spiritual facets, and believe the emphasis needed on any one of those facets is unique to each individual.

By scheduling this appointment, I attest to the following statements as part of my care agreement with Foresight Mental Health: ADHD Testing: I understand that validated, objective ADHD testing is required before initiating or continuing stimulant treatment at Foresight. I acknowledge that external records must be reviewed and approved, and that Foresight offers IVA-2 testing for a self-pay fee if needed. Medication Management: I understand that the decision to initiate, continue, or adjust medication will be made by the psychiatric provider following a full clinical assessment and is not guaranteed. Medication-Assisted Treatment (MAT): I am not seeking MAT services (e.g., Suboxone, Methadone) for opioid use or withdrawal, as these services are not offered by Foresight. Suicidal Thoughts: I confirm I am not experiencing active suicidal thoughts with intent or plan. I understand that if my symptoms indicate a need for a higher level of care, I may be referred to more appropriate services. Telehealth Access: I have a stable internet connection and a private space for telehealth sessions. I also have a backup option (such as a phone or hotspot) in case of internet issues. State Residency: I confirm that I currently reside in the state where I am seeking care and will be physically present in that state at the time of my appointments. I intend to remain in that state for at least the next 30 days to support continuity of care. No-Show & Late Cancellation Policy: I acknowledge Foresight’s 48-hour cancellation policy and understand that late cancellations or no-shows will result in a fee of $125 for therapy or $150 for psychiatry. I understand that repeated violations may result in discharge from care. Custody Documentation: If seeking care for a minor, I acknowledge my responsibility to provide current and accurate custody documentation if applicable. I understand that providing false or incomplete information may affect the child’s care and result in liability.
