What information about my appointment does the doctor or office receive?

Learn what information your doctor or their office receives when you make an appointment.

For every appointment Zocdoc sends over:

  • Legal first and last name

  • Date of birth

  • Sex

  • Reason for visit

  • If you are a new or existing patient

  • Phone number

  • Insurance carrier and plan

For imaging appointments, (ex. Mammograms, Xrays, CT scans, ultrasounds, MRIs, etc) Zocdoc also sends over:

  • Your referral order or contact information for your referring physician

  • Whether you’ve received prior-authorization from insurance (when applicable)

Some doctor's offices use a Zocdoc tool called Intake that will automatically share patient information with the practice before an appointment. If you are visiting a doctor whose team uses this, you will be prompted to share images of your insurance card and/or your ID as well as additional forms after you book.

If not, you can choose to send additional information on your own, like:

  • Image of your insurance card

  • Member ID

  • Additional sex and gender information

  • Additional notes
