Learn how to use Zocdoc's Intake solution

Intake is Zocdoc’s time-saving feature that allows you to send intake requests directly from your Inbox. Use it to collect important information from your Zocdoc and non-Zocdoc patients before their appointments.

Requesting Intake information

When creating a new request, select the Send patient intake button and then select the patient type.

If you select New or Existing, this will send the patient the specific Intake tasks you’ve laid out in your preferences.

Choose the Custom patient type to request additional based on the appointment specifics (e.g., visit reason, pediatric vs. adult appt., etc).

When Intake requests are sent

Immediately after you send the request, non-Zocdoc patients will receive a secure link (via email and or SMS) to submit their Intake information online. Your Zocdoc patients will receive their request automatically at the time of booking.

Patients who do not complete their tasks will receive 3 reminder messages: 48 hours, 24 hours, and 90 minutes before the appointment.

Preview what patients see

Send an Intake request to yourself to get a picture of how patients complete forms and upload their insurance and ID cards. From your Inbox, select Send patient intake, then select New as the patient type, and enter today’s date as the appointment date.

How to delete an Intake request

You can delete an Intake request by opening the request, then clicking the three dots in the top right corner.

By deleting, you'll remove any information submitted by patients from Intake and stop all reminder and patient confirmation messages for all incomplete Intake requests.

Viewing Intake Submissions

Stay on top of new submissions

When a patient submits their intake information, you’ll be notified via email and Inbox. Bookings with new intake will have a "Intake submission" tag in the Notifications column. You can also see all bookings with new intake submissions by clicking on Intake submissions on the left. Learn more about how to view submissions.

Easily view Intake submissions

To view the intake submission, click on the booking in Inbox or your calendar. You can then click on View details or Download in the "Insurance" or "Patient submissions" sections.

After you click View details or Download, the "Intake submission" tag in the Notifications column will disappear. You can still see all of your bookings by clicking All on the left, or viewing the booking in your calendar.

Quickly process patient information

Intake submissions will be available in Inbox for 1 hour after the appointment, or in your calendar for up to 60 days after they were submitted. If you need access to this information after 60 days, download and upload them to your EHR. This information will be directly uploaded to select EHRs.
