Zocdoc Intake is a free and simple solution that collects insurance cards, intake forms, and IDs online from patients. You can use Intake with patients who book through Zocdoc, your website, or over the phone.

Patients do not need a Zocdoc account to complete forms, which makes it easy to use with Zocdoc and non-Zocdoc patients.

How can Intake help my practice?

What you can do with Intake

Send patient Intake requests: You can request patient forms, insurance and ID cards via text message and/or email from all patients, even from those who booked over the phone.

Send Intake requests to all of your patients—they don’t need a Zocdoc account to complete your forms. Patients booking through Zocdoc will automatically receive your requests.

View and download patient information: Your patient submissions will be available directly in your Zocdoc account. You can view and download your patients’ Intake submissions in the Patient submissions tab of your Inbox.

Additional ways you can use Intake:

  • Use your existing office forms: We’ll create digital versions of the same forms you use in the office for patients to complete from any device.

  • Set different requests for your new and existing patients: This way you can avoid asking existing patients to unnecessarily fill out forms they’ve already completed..

  • Automatically remind patients to complete their requests: Reminders are sent 48 hours, 24 hours, and 90 minutes before an appointment.

  • Easily download patient submissions and upload directly into your EHR: Get digital versions of forms, insurance, and photo ID images to upload directly into your EHR. Intake can be uploaded directly into select EHRs.
