Collect Intake from all your patients, no matter how they book

Sending Intake requests to all patients is free—and patients do not need a Zocdoc account to complete your request.

NOTE: Automated intake to non-Zocdoc patients is in closed beta with select EHRs. While we are not accepting more clients at this time, please complete this form to join the waitlist.

There are three other ways to request intake information from your non-Zocdoc patients:

  1. Send patients their intake from Inbox: Click on Send patient intake from Inbox to request intake from patients who did not book on Zocdoc.

  2. Add the Intake form link to patient communications: See how to find your practice's intake link.

  3. Share a QR code in office or via patient communications: Reach out to our team to find your QR Code.

When the patient submits intake, you will see an "Intake submission" tag appear in the Notifications column of Inbox. You can also view all bookings with new intake by clicking on Intake submissions from the left side bar. Learn more about how to view patient intake submissions.
