How do I fill out my intake forms online?

If the office uses Zocdoc’s Intake product, after booking you’ll be prompted to fill out additional forms, images of your insurance card and /or ID.

You’ll also have the option to save this information in your Zocdoc account to autocomplete future forms, so you can skip the paperwork.

Where do I find the intake tasks?

After booking, you will have the option to complete your intake tasks on Zocdoc.com or on the Zocdoc app. From the Zocdoc website: click on your name on the top right. Then, select My Zocdoc or click on this link. From there, you will see a section called Upcoming appointment. Click on Prepare for appointment next to the appointment. This will take you to your Appointment Checklist.

From the Zocdoc app: click on Appointments on the bottom of the screen, then tap on the upcoming appointment. You’ll be able to click on the task in your Appointment Checklist.

When should I complete my intake tasks?

You should complete this information as soon as possible so that the office can better prepare for your visit and verify your insurance coverage and benefits.

Why am I receiving reminders to complete my forms?

You will receive text and email reminders until you complete your required Intake forms for your appointment. You can also log into Zocdoc and begin completing your forms for your upcoming appointments.

Can I save my information to Zocdoc to fill out my information faster next time?

Yes. If providers use Zocdoc’s Intake tool, you can save your information to Zocdoc to autocomplete for future appointments.

Can I change the information that Zocdoc autofills?

Yes. Zocdoc will pre-fill your forms using your saved information, which you can edit and update when completing any Intake tasks prior to your appointment.

Can I opt-out of allowing Zocdoc to save and autofill my information?

Yes, when completing Intake forms, you can choose not to save and autofill your information. Or, you can go to Settings > Permissions > Autofill within your Zocdoc account on desktop or mobile web to opt in or out. You can download a copy of your saved information as well.

Why am I unable to submit my intake form?

Make sure you complete all required fields, which are denoted with a "Required" tag. That is the most likely reason you are seeing this error.

I filled out the intake form. Did the practice receive it?

After you submit your Intake, all of your tasks will appear as complete on your Appointment Checklist and the practice will be notified. When you arrive in the office, you can also let the practice know that you filled out the information on Zocdoc.

I'm seeing an error when I try to upload my insurance card or government ID.

Only PNG and JPEG files are accepted at this time. If you are seeing an error, please make sure your file is converted to PNG or JPEG.
