Went in to get a disability form signed for Clipper Access (AKA Regional Transit Connection [RTC] Card). No issues whatsoever, and they did not gate-keep permanent disability status, unlike some doctors/healthcare institutions (like Kaiser...). Highly recommend.

It was a convenient way to obtain medical clearance but I was confused receiving emails from both zocdoc & doctor’s office. I would rather connect directly with doctor only after appointment is confirmed. Dr. Habbal explained the process, which I appreciated, he was very professional.
It may be helpful to offer recommendations such as local searches for private clinics providing bloodwork and/or EKGs. I had no idea what to search for as a cash paying patient to obtain tests.
