How much does this cost?
Intake is a new free tool that is included as part of your Zocdoc account. There is no additional charge for collecting insurance and ID cards, and your office forms from all of your patients.
Can I use my own office forms?
Yes! Send us your forms and we can create digital versions of the forms you are using in your office. Intake supports medical history, demographic, and consent forms, including forms with e-signatures.
Can I send requests to all of my patients, even those who call to book?
Yes! You can request patient forms, insurance and ID cards from all of your patients, including those who call to book. Patients booking on Zocdoc will be sent your requests automatically. Patients do not need a Zocdoc account to complete their forms.
Can patient responses integrate with my EHR?
Not at this time. However, you can download patient submissions and save it to your existing EHR, making your entire intake process paperless.
How long will this take me to implement?
Submit your forms and sit back. We make digital copies of your office forms within 2-5 business days. You’ll see the Intake tab in your Zocdoc account and will be able to start sending intake requests to patients. We automatically send patients booking on Zocdoc your request at time of booking.
Does this work with my existing patient communications tool?
Yes! You can include your practice's unique link in your existing communications with patients.
The providers in my office use different forms. How many sets of forms are supported by Intake?
We currently support one set of forms from each practice.
Do you support multi-language forms?
Only forms in English are supported at this time.
