Understanding your appointment card

Learn more about how to find patient information on your appointment card.

Select a booking in Inbox or your calendar to display the appointment card and view essential patient information, like images of their insurance card; at the top, you can also see the date of birth and if the patient is a new or existing patient.

The appointment card is grouped into several sections. The sections will be expanded based on what information is most relevant when you are looking at the booking.

The Insurance and Patient submissions sections will have a “New submissions” tag and automatically expand when a patient submits new information.

Appointment card actions

There are several actions that you can take in the appointment card via the icons in the top right:

  • Add a note to keep track of follow-ups by clicking the clipboard icon

  • Report patient behavior by clicking the flag icon

  • View the booking in your Calendar by clicking the calendar icon

When you click the 3 dots in the top right, you will be able to:

  • See patient record to view a patient’s past appointment history on Zocdoc

  • Print appointment card

Appointment details

In this section, you will be able to find the appointment time, visit reason, provider, location, and any notes that the patient has left regarding the appointment.

Insurance

The Insurance section is where you can find the patient’s insurance information. This section automatically expands if the patient submits new information; you will also see a “New submissions” tag.

You’ll also find the patient’s insurance card and when it was last submitted. For all bookings, the appointment card will show an insurance carrier and plan, or be marked as “Self-pay” if the patient selects “I’m paying for myself.”

Demographic information

In this section, you will be able to see the patient’s age and date of birth, sex, gender, and address if provided.

Sex & Gender Information

You can find the patient's sex and gender information within the Demographic information section of the appointment card. If you have a read-write calendar, you will see "Sex" in the patient record of your EHR; any additional sex and gender information will appear in the "notes" field under "gender."

Name, date of birth, and sex (as listed on a patient’s health or insurance records) will always be passed to assist with insurance or patient record matching. Patients may opt to share additional sex and gender information with providers, which will appear under “gender” in the appointment card or within the notes field of certain EHRs.

Additional sex and gender information, may include the following:

  • Assigned F at birth: a person who was assigned female at birth

  • Assigned M at birth: a person who was assigned male at birth

  • Genderfluid: a person who does not identify with a fixed gender

  • Genderqueer: a person who does not follow binary gender norms

  • Intersex: a person born with traits, including genital anatomy, reproductive organs, hormone function, and/or chromosome patterns that may not fit the typical definition of male or female

  • Non-binary: umbrella term for a person whose gender identity lies outside the gender binary

  • Transgender man: a person whose gender is male and whose sex assigned at birth was female

  • Transgender woman: a person whose gender is female and whose sex assigned at birth was male

Contact information

Here, you will find the patient’s phone number and email address. Zocdoc verifies all patients’ phone numbers before they can book appointments. Learn more about the different ways that you can contact a patient.

Note: If an appointment was booked for someone else (e.g., a user's family member) and their email was included, you will see two emails listed. The email will be the email of the patient you will be seeing, and the contact email will be the email of the person who booked on their behalf.

Referral

If applicable, you will be able to see the patient’s referral order. Learn more about referral orders.

Patient submissions

The Patient submissions section is where you can view and download ID card images and your completed office forms. If you have patient confirmation status enabled, you can also see whether the patient confirmed their attendance. Learn more about patient confirmation status.

We recommend downloading insurance cards, forms, and IDs before the appointment, as bookings will be removed from Inbox one hour after the appointment time. Learn more about how to request IDs and office forms.

This section automatically expands if the patient submits new information; you will also see a “New submissions” tag.

The Booking activity section shows more information on how the appointment was booked and when.
