Please be sure to contact the provider via text before the visit to confirm your insurance/copayment or self pay fee. If you need to cancel your appt, please do so within 24 hours of initial booking. Please verify your deductible, co-payment /coinsurance responsibility, familiarize yourself with your insurance benefits- Aetna, United Healthcare plans and Medicare Part B original accepted. You are welcome to use HSA, FSA or credit card for co-insurance, co-payment or deductible payments. If you are self pay - payment is due prior to visit $100 initial visit. I currently provide telehealth primary care services to patients located in California only. Once appt is confirmed, you will receive access to SimplePractice patient portal to complete intake questionnaires and payment method prior to the visit. Please note some health plans do not cover telehealth physical exam/annual wellness exam -please verify with your insurance prior to the visit.
My goal is to listen to patient's concerns to provide quality care. I focus on prevention, timely screening and treatment.
