Oparin Medical is a self-pay virtual direct primary care practice. Full payment of $200 is required in advance to confirm your first appointment. This includes the initial 60-minute video consultation, a written assessment and care plan, and the first month of follow-up. I offer concierge-level service through unhurried consultations, direct physician communication, and continuity of care. I am a board-certified family physician with 29 years of clinical experience in Russia, South Africa, and the United States. I also hold board certifications in obesity and lifestyle medicine. My work includes preventive care, chronic disease management, weight management, and the evaluation of complex health concerns. I take time to understand your medical history, review previous investigations and treatment, and develop a practical plan that reflects your needs and priorities. Please complete the required intake and consent forms before your visit. Your first month begins on the date of your initial consultation. Membership renews automatically at $200 per month and includes ongoing messaging and scheduled video follow-up when medically appropriate. You may cancel before the next renewal. I do not bill insurance; medications, laboratory tests, imaging, and services provided by other clinicians are charged separately. This practice is intended for patients seeking thoughtful evaluation and an ongoing relationship with their physician. It is not an urgent-care or emergency service. I do not prescribe opioids or provide opioid pain management. All prescribing decisions depend on an adequate evaluation, review of relevant medical records, and appropriate follow-up. One-time visits booked solely to obtain an immediate controlled-substance prescription or refill are not appropriate for this practice. Booking and payment do not guarantee a particular medication or a same-day refill. Some concerns require an in-person examination, testing, or referral. Care is available in English and Russian for patients age 12 and older who are physically located in California or Kentucky during their appointments. I am currently unable to accept patients enrolled in Medicare, including those wishing to pay privately.
