Pricing:
The cost of each visit ranges from $99.00 to $169.00 for primary care visits depending on the appointment type and needs of the patient.
You will receive a separate invoice for the cost of your visit from Intuit/Quickbooks. This should be paid at least 24 hours prior to your appointment to secure your appointment slot. If for some reason you pay for your appointment and have to reschedule, we will apply that payment to your next scheduled appointment. If you have any questions feel free to message me and I will get back to you as soon as possible.
Booking/Confirmation:
If bookings, you will receive an appointment confirmation email from our practice system Patient Fusion. In the email, there will be an orange button that says Forms. When you click there, you will be taken to the intake form to complete. This MUST be completed at least 48 hours prior to your appointment start time to give your provider time to review it or your appointment may need to be rescheduled.
No Show Fees:
For any no show appointment not cancelled in ZocDoc, a fee of $50.00 will be assessed.
