We ONLY accept Medicaid of Utah and NO OTHER Medicaids. We DO NOT accept HMO plans.

Here are a few important things to know:
Intake Process:
1) Please complete the intake forms as soon as possible.
2) Expect a confirmation call from us at 412-467-6410. During this call, we will collect information from you to ensure a successful first appointment and set up your patient portal account.
2) If we are unable to reach you prior to your first appointment, the first 15 minutes of your initial appointment will be with our administrative staff to complete your intake.
3) Self pay fees or estimated insurance fees are due 24 hours prior to your appointment start time. If we are unable to reach you prior to your first appointment, please have your form of payment ready at your initial appointment. If payment is not made, your appointment may be cancelled.
4) Your first video appointment will take place in ZocDoc. If you have any questions or concerns, you may call or text us at 412-467-6410.
Insurance:
1) We are a multi-state practice with our in network insurance being through Oregon. This means that our in network status may vary based on your individual insurance. It is important that we receive your insurance information prior to your appointment so that we can verify the in network status and provide you with an estimated cost of services.
2) Unfortunately we are unable to accept any Medicaid or Medicare plans. If you have a Medicaid plan, you may self pay. Due to federal regulations, we cannot accept self payment if you have a Medicare plan.

Douglas Ludington provides comprehensive primary care. While he can assess various conditions and refer you to specialists when needed, please understand that he is not a specialist.

When I tried to dial in, the appt gave me problems and made me about 5 min late.
But the lady said shed tried to get hold of me for a while, and let me know they aint.taking cigna insurance. The commo seemed to work for me.
