Rates are as follows: Initial 50 Minute Consultation Session: $800.00 Returning/Ongoing 45 Minute Session: $415.00-$600.00 60+min Sessions: $600-675.00 No Show/Late Cancellation: $415.00-$600.00 ============================================================================ Please be aware that at this time, Balanced Psychiatry of New York does not prescribe any immediate Controlled Substances or Narcotics. Examples of the medications that are not prescribed include: Xanax (Alprazolam), Klonopin (Clonazepam), Adderall (Amphetamines), Ritalin/Concerta (Methylphenidate), Ambien (Zolpidem), etc. ========================================================================= Our office must receive your intake, treatment consent, and health history information on Zocdoc in order for us to proceed with your appointment. **If forms are not completed in time, 15-20 minutes of the initial appointment will need to be spent reviewing and signing all documents, similar to medical appointments. If you require additional time to complete the paperwork please feel free to reschedule your appointment using Zocdoc to allow a day or two to complete your paperwork. Please be aware that without these completed forms, while our meeting will still take place as scheduled I will be unable to initiate treatment or issue any prescriptions during the consultation. It is important that the treatment consent forms, practice registration forms, and ideally, the confidential health history form be submitted prior to your appointment.
