In order to provide you with the highest level of personalized treatment and service, Camber Dental accepts most insurance carriers as an out-of-network (OON)/open provider. As a complimentary service to our patients, we will submit a predetermination of insurance benefits and eligibility upon request. Please keep in mind that while Camber Dental can assist in determining your estimated eligibility and benefits, payment is required for all services in full at the time of treatment. We will submit a claim to your insurance carrier on your behalf, or provide you with a complete, itemized invoice that you can submit to your insurance company for any approved OON reimbursement. We strongly encourage all patients to verify their insurance benefits and eligibility directly with their insurance provider prior to their appointment.
