Continuum Integrated Mental Health

Continuum Integrated Mental Health provides personalized psychiatric care for individuals seeking greater emotional stability, clarity, and overall well-being. The practice was founded on the understanding that mental health and healing exist along a continuum. Progress is not always linear, and each person may require different levels of support during different seasons of life. The practice takes an integrated, whole-person approach that considers emotional health, physical health, relationships, life experiences, culture, environment, personal values, and spirituality when welcomed by the patient. Care is compassionate, trauma-informed, culturally responsive, and grounded in current clinical evidence. Services include comprehensive psychiatric evaluations, medication management, supportive psychotherapy, ongoing symptom monitoring, and coordination with therapists, primary care providers, and other members of the patient’s treatment team when appropriate. Treatment may address concerns such as anxiety, depression, ADHD, trauma-related symptoms, mood changes, difficulty sleeping, stress, grief, life transitions, relationship challenges, and problems with concentration or motivation. Appointments are collaborative rather than one-sided. Patients are encouraged to ask questions, share concerns, discuss previous treatment experiences, and participate actively in decisions about their care. Medication may be considered when clinically appropriate, but it is not viewed as the only path to improvement. Treatment recommendations may also include psychotherapy, behavioral strategies, lifestyle changes, coping skills, improved sleep practices, medical follow-up, or additional evaluation when needed. Continuum Integrated Mental Health recognizes that many people appear capable and successful on the outside while privately feeling overwhelmed, anxious, emotionally exhausted, unfocused, or disconnected from themselves. The goal is to provide a respectful space where patients feel heard without judgment and can better understand the patterns, symptoms, and experiences affecting their daily lives. The practice is led by Takiyah Jones, a dual-certified Family Nurse Practitioner and Psychiatric Mental Health Nurse Practitioner who has a Doctorate in Nursing Practice. Her background includes psychiatric care, primary care, nursing education, crisis support, substance use treatment, community-based care, and work with individuals experiencing complex mental health needs. This combined experience supports an approach that considers both mental and physical health rather than treating symptoms in isolation. Continuum Integrated Mental Health is committed to helping patients build insight, strengthen coping skills, improve functioning, and develop a treatment plan that feels practical, individualized, and sustainable. The purpose of care is not to define a person by a diagnosis, but to help each patient move toward greater stability, resilience, connection, and hope.
