Core Primary Care
Credit Card on File: Starting October 2025, we collect credit cards on file to streamline visits and billing. This is required for all patients by January 1, 2026. Cards help ensure faster check-in, simpler billing, and improved scheduling efficiency.
Booking policies:
1. If we cannot get ahold of you within 24 hours of you booking your appointment, your appointment will be forfeited.
2. If you reschedule within 24 hours of your appointment time, a $35 late-rescheduling fee may be applied.
3. If you no-show to your appointment, a $50 late fee will be charged.
