Cotallat Psychiatry
Welcome! Before your appointment, someone from our office will call you to do a brief screening to make sure we’re a good fit for each other, as some mental health conditions may require a higher level of care than we can provide remotely. Once we confirm that, you’ll receive a link within 24 hours to complete your New Patient Forms through our platform, Simple Practice. You’ll also get a separate link from Headway, the platform that handles our billing and insurance. Both need to be completed before your scheduled visit.
•Cancellations less than 24 business hours’ notice will result in a charge not less than $100 and up to the full session fee of $300.
•No-shows or missed appointments without proper notification will also incur this fee.

Mr. Cotallat is an absolute gem. I have been dealing with personal issues that could be crippling at times, for my physical and emotional health.
