We proudly offer Annual Exams, Acute or Chronic visits, Men's, Women's and Adolescent Health, Concierge Medicine, Televisits, Therapeutic Injections, Joint Injections, Derm procedures, and more for patients within the Dallas/Fort Worth and surrounding areas. ****Our office requires Patient Intake forms, Front back copies of insurance and Drivers License at the time of booking. Please be advised that Dr. Cornelius does not see patients younger than 13 years of age. All Zocdoc bookings are Telemed appointments only. Last, our practice is currently not accepting HMO Plans, Self Funded Plans, or Obama Care for new patients.
