Dana Curley Nutrition
Helping you love your food & reach your health goals: start where you are. I believe that good nutrition is attainable through personally tailored, evidence-based, doable, accessible, dietary changes. Building a sense of partnership with clients is the key to our success. I specialize in weight management, diabetes, prediabetes, cardiovascular concerns such as cholesterol and high blood pressure, autoimmune disorders, and GI issues including IBS, Crohn’s, lactose and gluten sensitivities.
We recommend that all patients contact their insurance company to verify coverage and benefits prior to services delivered. If a referral is required, we will work with your provider to obtain one.
Note that Medicare only covers nutrition counseling for diabetes and kidney disease.
If medical nutrition therapy is not a benefit of your insurance plan, per billing policy, you are responsible for paying the self-pay rates of $200 for initial session.
NOTE: You can use an FSA/HSA account to pay for co-pays or self-pay costs incurred.
Questions to ask your insurance provider:
1. Does my plan cover medical nutrition therapy? (CPT codes 97802/97803)
2. Is a referral required for medical nutrition therapy services?
3. How many visits are allowed per year? How many preventive? Medical?
4. Do I have a deductible I need to meet?
5. Is there a co-pay or co-insurance for each visit?
6. Does my health insurance plan cover telehealth?
