Elam Health and Wellness is a physician led practice directed by Dr. Stephanie Opusunju, focused on long term health outcomes through preventive and metabolic health, hormone optimization, mental health, and general internal medicine. Appointments booked through Zocdoc are $315. Direct booking at Elamhealth.com is $295. A limited number of insurance plans are accepted and patients are responsible for applicable copays. Concierge care options are available for those interested in a more comprehensive level of support. Appointments include evaluation and management as well as appropriate medication refills, typically up to a 3-month supply when clinically indicated. After booking, patients will be contacted by the office to confirm details and will receive an email invitation to create a patient portal account. The portal is used to complete your health profile, upload documents, and manage follow up care. Please note the following: * Refills for controlled medications are provided on a case by case basis after clinical evaluation * Medications requiring ongoing monitoring such as lithium or isotretinoin are not prescribed * Administrative forms, including FMLA and similar documentation, are completed for a separate fee and require a visit.
