We operate under a Direct Primary Care (DPC) model, which means instead of billing insurance, patients pay a simple monthly membership fee. This allows for more time with your provider, better access, and care that focuses on the root cause—not just quick fixes. What is Direct Primary Care (DPC)? Direct Primary Care is a membership-based approach to healthcare that removes the middleman of insurance. With DPC: No copays No surprise bills No rushed appointments Direct access to your provider Instead, you pay a flat monthly fee that covers your primary care needs. What’s Included in Membership Your membership gives you access to: Comprehensive primary care Same-day or next-day appointments (when available) Longer, unrushed visits Direct communication with your provider via text, messaging, or telemedicine Preventive care and chronic disease management Weight management support Hormone replacement therapy (HRT) At Elevate Wellness, everything is approached through a whole-person, root-cause lens. That means we don’t just treat symptoms—we look at the bigger picture of your health. Telemedicine Membership Details We currently offer telemedicine-based care for patients located in: Texas Colorado Utah Membership pricing: Adults: starting at $50/month Children (ages 2–17): reduced monthly rate when enrolled with a parent Family plans available There is a one-time enrollment fee. Your membership covers care provided directly through Elevate Wellness.
