New Patient Consultation $350 INCLUDES TWO (2) VISITS - one (1) - 60 minute initial visit - one (1) - follow up visit for treatment, medications, and lab review within 30 days of first visit Elitist Membership $150 per month - After New Patient Consultation, one (1) routine visit per month $75 each additional visit - Priority scheduling - No charge for prior authorizations review and completion 20% discount on form review and completion (FMLA, disability, medical) - Easy access and and open communication via phone, patient portal, and emails Fee for Service Visits (non-members) $200 per visit - After New Patient Consultation, each follow up visit is a charge of $200 - Fee for all additional visits with no membership - Prior Authorization - $25 per prior authorization - Forms (such as FMLA or disability paper work) - $150

What an amazing experience! Dr Sainvil surpassed my expectations and really set the bar high. I’ve never had a physician who truly cares and actually takes the time to listen.
