Ember Life Psychiatry
In order for patients to be seen, we need to receive your insurance and intake information on Simple Practice (SP) platform and Headway. All consent forms/documents are needed prior to the appointment time. You will receive an email to sign up to the platform and an email from Headway (use for insurance services). Please complete the forms as soon as possible to delay any further appointment scheduling. Virtual visits are only on SP.

Please note: Out-of-Network and Self Pay Rates are as follows: Net patient visits $200 and follow-up visit are $150.
-Please call the office for the self-pay rate. 404-905-7229. We do charge the fee before the appointment (initial and follow-up visits) within 48 to 24 hours ahead of time.
