We are truly a team dedicated in promoting the best standard in care and utilizing evidence-based practice to better health or sickness. The Primary Care is lead by a Board-certified Nurse Practitioner. We listen to your concerns and provide the best holistic care to better your health. Conditions Treated:
-Preventive Services Vaccination
-Disease Diagnosis through physical exams, diagnostic test such as X-Ray, Laboratory, EKG, etc.
-Common illness such as Cold, Flu, Sore-throat, Cough, and Nasal Congestion.
-Chronic Disease management such as Diabetes, Hypertension, COPD, referral to specialist with severe conditions and follow up on specialist feedback.
-Annual Physicals is a yearly checkup that is an important tool for everyone. Even if you are healthy come to your Doctor once a year.
-Screening and Treatment for common age related condition such as Diabetes, High Cholesterol, High Blood Pressure, etc.
-Medical Weight Loss Management with or without insurance. We charge moderate for out of pocket pay

Booking Policy:
-If you book an appointment through Zocdoc and want cancel, it must be within the first 24hrs. Any cancellation after 24hrs of booking will be charged $50 fee

Co-payments are expected during your visit.
If you are a self-pay patient or if you are subjected to a deductible that has not been met, payment is expected at the time of service.
If you have an insurance change please bring your new insurance card to your visit.
