ATTENTION: MANDATORY CONCIERGE MEMBERSHIP REQUIRED Annual Membership Fees: Individuals (Age 26+): Starts at $600 Quarterly (or $2,400 Annually) Individuals (Age 6-25): Starts at $300 Quarterly (or $1,200 Annually) _ At Family Medicine Shady Grove, our mission is to provide cutting-edge, personalized medical care to help our patients live happier, healthier lives. Our concierge practice ensures you receive the exceptional medical attention you need through tailored therapeutic services. *IMPORTANT INFO*- 1. New patients- Please enroll for the Concierge practice by calling 301-664-2835 or at www.shadygroveenroll.com BEFORE REQUESTING APPOINTMENT. Please let us know if you would like a Meet and Greet as a new patient before you sign up and we can schedule it with you. Immigration physical exam patients DO NOT need to sign up and can request appointment. 2. If you cannot make it to your appointment please give us 24 hours notice to cancel your appointment so we can offer it to someone who really needs it. 3. When scheduling for your physical please make sure it has been a year since your last physical or check with your insurance plan if they would cover if under a year, and come fasting for atleast 8-10 hours (nothing to eat but drink 16-24 oz of water). We welcome you to our practice and excitedly look forward to providing you with personalized care and guide you in your health journey!
