Thank you so much for considering NP Zak for your mental health needs. Svetlana Zak has been in the mental health field over 20 years and has practiced psychiatry as a Nurse Practitioner for over 14 years. She was born in Moldavia and grew up in Brooklyn, NY. She speaks fluent English and Russian. Her expertise is ADHD, Mood and Anxiety Disorders and most other mental health conditions. IMPORTANT: After you book your appointment on Zocdoc there is an intake process.
1. Respond to an email from billing and enter your insurance and copay information.
2. Fill out your psychiatric intake: brief medical history, psychiatric history, allergy information.
3. Since you will have to fill out the intake, Zocdoc check in is NOT REQUIRED. You DO NOT need to add your insurance cards. Simply provide the information when you get your email from the billing company.
4. The VIDEO LINK will be sent to your email and your phone by text message prior to the appointment.
***ZOCDOC gives us 24 hours to confirm your appointment. Please respond to all emails promptly so we can process your request.
What if we don't take your insurance? We can provide a SUPERBILL that can be submitted to your insurance plan for reimbursement. You can pay for the appointment with a credit card or VENMO us. Just let us know and we will assist in booking. Have a great day and look forward to meeting you.

Dr. Zak’s appointment process was quick, easy, and precise. I felt that she listened and understood what I was communicating with her.
