I saw the doctor in September via video visit and was told that she would be sending me some ADHD literature and other resources that she felt would be useful, but I never received anything. I was also told she would be reaching out to my old provider in AZ to verify my previous treatment plan but after connecting with their office, I was told that she had not done so either. She told me that I could contact her with any issues with medication, but I haven’t been able to reach her at any of the numbers or email addresses that I’ve located online. She doesn’t have any appointments available on Zocdoc or in any other easily accessible place online, so I have had to reach out to another new provider to receive treatment for my conditions. I liked the doctor, but I’m very disappointed at the lack of follow-up or any communication whatsoever.

The appointment was nice. It’s the follow up that I have an issue with. I tried calling after my appointment to get my clinical notes and …nothing. No reply, no response and no follow up. All I want is my visit notes.
