If this is your FIRST visit with us, please be advised that an IN OFFICE visit is REQUIRED to perform a dermatology, physical, pap, or ADHD consult, this includes a rash as you must be assessed by a provider.YOU MUST BRING A COPY OF YOUR INSURANCE CARD WITH YOU TO BE SEEN! A no show fee of $100 will be charged for any appointment that is missed or not cancelled 24 hours ahead of time.
Any controlled substances need to be seen in office. If you are not sure if you need to be seen in office for your current visit you are scheduling, please call our clinic at (303) 953-9405 to check. Follow up visits may be scheduled as a telehealth for these concerns depending on provider discretion.
