We are excited to provide your health care and appreciate the opportunity to be part of your wellness journey. Before scheduling your appointment, please take a moment to review our practice information, including our services, office hours, and important policies.
Services & Pricing
We proudly offer comprehensive primary care services for patients of all backgrounds in a welcoming and inclusive environment.
For patients without insurance, we offer transparent, affordable self-pay pricing:
* New Patient Consultation: $115
* Comprehensive Annual Physical: $230 (includes the office consultation)
We also accept most major insurance plans.
Please complete all patient forms at time of scheduling your appointment. All required forms must be completed no later than 24 hours before your scheduled appointment. If your forms have not been submitted by this deadline, your appointment may be canceled or rescheduled to allow sufficient time for review and preparation by our clinical team. If you have difficulty completing your forms, please contact our office before your appointment so we can assist you.
Appointments that are not canceled or rescheduled at least 24 hours in advance are subject to a $65 no-show fee.

Laboratory Hours
Monday: 7:00 AM – 4:00 PM
Tuesday: 8:00 AM – 5:00 PM
Wednesday: 7:00 AM – 4:00 PM
Thursday: 8:00 AM – 5:00 PM
Friday: No phlebotomy services available
*NOTICE: Routine physicals scheduled outside of our lab hours will have laboratory work scheduled for another time.
Insurance Information
While we are contracted with many insurance plans, insurance benefits and coverage vary significantly between employers and policies.
It is the patient’s responsibility to verify:
* That InclusiCare Medical participates with your specific insurance plan.
* Your eligibility and benefits.
* Any copayments, deductibles, coinsurance, or services that may not be covered.
Because your insurance policy is a contract between you and your insurance company, we cannot guarantee coverage or payment for any specific service. Patients are ultimately responsible for any charges not covered by their insurance plan.
Controlled Substance Policy
InclusiCare Medical is committed to the safe and responsible prescribing of medications.
As part of this commitment:
* We do not prescribe Schedule II opioid medications.
* We prescribe controlled substances only when medically appropriate and in accordance with applicable federal and state regulations and evidence-based guidelines.
* Patients receiving ongoing controlled substance prescriptions may be required to sign a Controlled Substance Agreement, complete periodic urine drug screening, and attend regular follow-up appointments, typically every 30 days.
Our prescribing decisions are based solely on patient safety, medical necessity, and current standards of care. We encourage patients to discuss any questions regarding this policy with their healthcare provider before establishing care.
