IY NP in Adult Health PC is a patient-centered practice dedicated to providing comprehensive psychiatric and adult primary care services with a focus on whole-person wellness. Led by a dual board-certified Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) and Adult/Geriatric Nurse Practitioner (NP-BC), the practice is uniquely positioned to address both mental and physical health needs in an integrated, collaborative manner.
Our philosophy is rooted in the belief that optimal health requires attention to both mind and body. Mental health conditions such as anxiety, depression, trauma-related disorders, mood disorders, and attention difficulties often intersect with medical concerns including hypertension, metabolic disorders, chronic pain, sleep disturbances, and age-related health challenges. By treating these conditions together—not in isolation—we provide more accurate assessments, more effective treatment plans, and improved long-term outcomes.
We provide evidence-based psychiatric evaluation, medication management, and supportive psychotherapy for adults and older adults. Care plans are individualized and thoughtfully developed, incorporating biological, psychological, and lifestyle factors. When appropriate, we integrate medication management with behavioral strategies, lifestyle optimization, stress regulation techniques, and coordination with primary care or specialty providers.
For our adult and geriatric patients, we understand that health concerns evolve over time. We emphasize preventive care, cognitive health, chronic disease management, and safe medication practices, especially for patients managing multiple prescriptions. Special attention is given to minimizing polypharmacy and ensuring treatments are both effective and sustainable.
