Upon completion of booking, you will receive an email to set-up your electronic health record using our highly secure Athena Health Portal. Please create your profile and complete all required intake forms in order to keep your booking from being cancelled. These tasks need to be completed a minimum of 24-hours prior to your appointment time to avoid cancellation or re-scheduling.

I was able to get an appointment within a couple of days, which was great as I had some urgent needs. Scheduling, completing forms online and being seen all went smoothly and quickly. Convenient location and nice office. Dr. Naz was very professional, knowledgeable, yet also kind and personable, and I felt took a sincere interest in wanting to help me improve my overall health and resolve my long term issues. She took care of my urgent needs during my first visit and outlined a plan of action moving forward to address my other concerns. This appointment was for my 11 year old daughter. Naz made her feel comfortable and invited her to share her own concerns rather than deferring to me (mom) right away. I appreciate that, as I believe it’s important for kids to start learning how to advocate for themselves early in life.
